Registration Package

How to register:

1.  If paying by credit card (Visa/Mastercard only), please go to the registration link on our website http://sailparrysound.on.ca/sps-sailing-school/registration/

2.  If paying by cheque:

§  fill out a REGISTRATION FORM (Page 2 & 3) for each student. It is a table within this Word.doc which can be completed on your computer and emailed to .

§  Fill out the MEMBERSHIP FORM (Page 4) for your family, also within this Word.doc which can be filled out on your computer and emailed to

§  Print and sign the MEMBERSHIP FORM (Page 4), THE WAIVER FORM (Page 5) and the ISLAND TRIP PERMISSION FORM (Page 6) and mail all three signed forms with your cheque to: Sail Parry Sound, Box 734, Parry Sound ON, P2A 2Z1.

3.  Regatta/Race Information:

§  Students must have fully paid enrolment in sailing school the week of a regatta, in order to participate at regattas, at home or away.

§  Regatta registration is a separate process, not automatic, and fees are in addition to sailing school fees.

Confirmation:

Registration is confirmed on receipt of forms and payment in full. In case of a tie, your emailed forms time and date will be the deciding factor. You will be contacted by email with confirmation, and your receipt will be emailed to you.

Subsidy:

Because SPS is a certified High Five program, our sailing school is eligible as a day care option for government assistance to families. Details are at

Membership is a prerequisite for Sail Parry Sound Sailing School; fee for your family is $85 a year.

Waiver and Signature: In consequence of being a member and taking part in the activities of Sail Parry Sound, Inc (SPS), an activity that involves physical risks, members assume the risk of participation and agree to waive any and all claims that they, their family members, their executors, legal representatives, heirs, successors and assignees may have against SPS, its committees or Officers for accidents, injury or death arising from participation. They also indemnify each of the above against any claim arising from participation by their guests, not herein named, in SPS events or at the SPS site.

WAIVER, RELEASE, ASSUMPTION of RISKS

Prior to participating in Sail Parry Sound Sailing School (SPS3), all participants in SPS3 must sign and return this waiver stating that they have read the conditions of the waiver and are aware of the risks associated with the event, and thus waive any right that they may have to any course of action against Sail Parry Sound Inc., its directors and their respective agents, officials, volunteers and representatives for injury to the person(s) or loss/damage to personal property.

1. I and my next of kin and my parent(s) / guardian(s) are aware that there are risks associated with all water sports, including the sport of sailing. Certain of the risks and hazards include: launching, docking and storing equipment used in the sport of sailing, high winds that may cause rough water, natural forces which may result in obstacles and hazards, cold water in which extended immersion can be life-threatening, and weather that may change rapidly and be extreme, presenting significant challenges. I or the undersigned parent/guardian hereby give notice that I am able to swim and that I voluntarily assume all risk of loss, damage or injury, including death, that may be sustained by me or to any property while engaged in said activities. *INITIAL______

2. Therefore, to induce SPS3 to accept his/her child into the Sailing Program, the undersigned parent/guardian agrees to hold harmless and indemnify Sail Parry Sound Inc., its officers, directors, employees and agents from any and all claims, losses, damages, fees and liability growing out of or in any manner related to injury to a person or damage to any property arising out of or in anywise connected with the operation of the SPS3 or any activities on or the use of any facilities or equipment of Sail Parry Sound Inc. *INITIAL______

SAFETY

3. I agree that I or my child will properly wear an approved personal flotation device at all times while participating in or near the water, and that I/we will follow the safety precautions and instructions prescribed by SPS3 instructors or designated personnel. *INITIAL______

PHOTOGRAPHIC RELEASE

4. I hereby acknowledge that I or my child may be photographed while participating in SPS3 activities and/or programs. I hereby unconditionally authorize Sail Parry Sound Inc., at its sole discretion, to use any such photographs in brochures, flyers, web site, and any other promotional or educational materials. *INITIAL______

PARENTAL AGREEMENT

5. I/we as parent/guardian understand that I/we are responsible for our child’s behaviour and conduct while at SPS3 and will see to it that our child adheres to the program rules. I/we agree to assume the obligation for expenses of repair and/or replacement of program equipment that is attributed to our child’s reckless or irresponsible behaviour. *INITIAL______

In signing the foregoing release, I hereby acknowledge and represent that I have read the foregoing release, I understand it and agree to it voluntarily, that I am the full age of majority or, in the alternative, I have indicated that I am the guardian of the minor participant named, and agree that this agreement will be binding on my heirs, next of kin, executors, administrators and successors.

ISLAND BEACH TRIP PERMISSION FORM

To the parent or guardian of sailing school students:

On Thursday of each week we are planning a day trip to one of the adjacent beaches on Parry Island. This trip is beyond our normal sailing boundaries and the boats may not be visible from Sail Parry Sound.

There will be constant supervision from our instructors and the sail plan will be posted on the NOTICE BOARD on the Boathouse.

The class will not go if the weather is not suitable.

It will require packing a lunch that the child will take on the boat, so it must be in a water- resistant package.

There must be unanimous consent for the class to go but you are not to feel any pressure to grant consent if you or your child are not comfortable going beyond our normal boundaries.

We hope that you permit your child to come with us because we always have a good time!!

Island Beach Trip Permission
