With signing this form, the signed person declares to agree on the following rules applying to this short course:

•The short course will tak place at BAU campus from 9 AM on December 11 until 5.30 PM December 15, 2016 (5 full days). The course participant commits to active participation.

•The course fee is 15000 BDT. In the edition of the 2016 course, ICF-BAU offers this course with 20% discount, resulting in a course fee of 12000 BDT for 2016.

•Once we have received this registration form and the full course fee, you are officially registered and you will receive an invitation letter with more detailed information about the course.

•The course fee includes: 5 days of training sessions from experienced experts from BAU and external resource persons, (hard copy and soft copy of) training materials, 5 times working lunch, 10 times tea/coffee and daily refreshments.

•The fee does not include: transport to and from the venue, accommodation, breakfast and dinner.

•The participant is supposed to organize for his/her own transport to the venue. It would be appreciated if the expected arrival and departure time of the participant will be communicated in advance to .

•The participant is supposed to arrange his/her own accommodation at the BAU Campus or in Mymensingh. ICF can provide linkages to appropriate accommodation if needed. In case you would need our support please contact us via .

•A large part of the curriculum consists of case studies and group work. Therefore it would be appreciated to timely communicate any foreseen absence to the course coordinator.

•Our cancellation policy is as follows:

Cancellation is free of cost when it is communicated at least 3 weeks before the start of the course
50% of the Course fee will be reimbursed if cancellation is communicated between 3 and 2 weeks before the start of the course
In case of cancellation is communicated less than 2 weeks before the start of the course, no reimbursements can be made.
