Purpose: SC-CHAP will work with local institutions to discourage discharging people into homelessness, encouraging proactive discharge planning and by facilitating access to prevention services and other continuum services. Institutional Discharges have been identified as one source of homelessness. The purpose of this document is to have our community’s hospitals and/or health clinics identify those that might be homeless upon discharge and to then provide such health care providers with information on community services available to prevent homelessness. The goal is to prevent discharge into homelessness (homeless shelters) unless absolutely necessary (a hospital discharge of a victim of domestic violence to the Underground Railroad domestic violence shelter, e.g.).

Policy: SC-CHAP will encourage local agencies which provide institutional housing to work with local institutions so people are not discharged from their facilities into homelessness.

Discharge Plan: Each hospital will determine the person or department that would be responsible for assuring that those admitted would have a discharge plan review to assure that any discharges into homeless shelters would be kept to a minimum and be the best placement possible. This will vary as the needs of the patients will range from out-patient to long-term care. The intent of the policy is to have such a review occur prior to the eminent discharge of all patients whenever possible.

If the discharge plan review indicates that the patient is without housing resources, the programs/services on the following page are to be called. These programs will attempt to assist the patient in garnering housing resources upon discharge.
