Application for Phytosanitary Registration of Production Units for Exports of Citrus Fruit from South Africa to China, EU, Iran, Japan, South Korea,Thailand , USA

NB:

  • Please read and understand all the requirements carefully.
  • Please also ensure that you are familiar and understand the requirements of the market you want to export your fruit to.
  • Section A & B must be completed in full
  • All producers should ensure that they indicate the varieties/cultivars and orchard numbers (orchard numbers should be alpha numeric-A1,C1,H1 etc) for all markets (EU, China, Iran, South Korea, Japan, Thailand, and USA) on section B.
  • Please do not use abbreviation for varieties/cultivars; please write them in full and should be as stipulated in the quality standard document
  • Annual tariff of R95-00per PUC per special market and clearly indicate the PUC/PHC on the proof of payments as reference;
  • All PUC registering for EU must complete APPENDIX A and attach it to this application form and keep the copy in the farm.
  • For South Korea, ALLPUCswhich exported in 2016can apply without motivation. New PUCs and PUCsthatapplied inspected and approved and did not export in 2016 need to provide a motivation and or proof of why they should be included for 2017 export season.
  • For South Koreamust complete APPENDIX B, and the copy of APPENDIX Bmust be kept on the farm for record purpose.
  • All the copies of registration documents should be kept in the farm for record purpose and should be available for DAFF officials at all times.
  • All application must be accompanied by Maps of the farm, Bactrocera dorsalis surveillance reports and GPS coordinates preferable taken

at the farm office. An application form without GPS coordinates will not be accepted.

2016/2017 SEASON

Declaration of Intent

I/We the undersigned as agent/ producer understand and shall comply with all the stipulations of the applicable export programme(s) with specific reference to the following:

  1. Only fruit produced in registered and approved orchards shall be sourced for the applicable special export programme(s).
  2. Consignments shall be free of any of the listed quarantine organisms/pests that pose a phytosanitary risk to the importing country.
  3. Only DAFFand PPECB approved packing houses and cold chambers may be utilised for handling, storing and cold sterilisation of the fruit.

4.Only fruit that are from the approved PUC and clearly marked for a specific export programme shall be allowed for export.

5.Application for registration of production unit shall not be considered if:

a).The PUC is not appearing in the current FBO update list;

b).The registration document is illegible (information shouldbe typed or completed in clear block letters);

c).Information is insufficient or incorrect;

d).The documentation is received after the closing date;

e).There is no proof of tariff payment.

6.Pages must be signed at the indicated spaces as required and initialed at the bottom of each page where space for signature is not provided.

7.DAFF shall not be held responsible for any financial or other losses resulting from non-compliance by the producer or the exporter agents providing incorrect, insufficient and/or illegible information.

8.I/We accept the fact that if in any manner I/we do not comply with the requirements of the applicable export programme (s), which include interception rate of quarantine pests, could result in expulsion from further exportof fruit to that specific market.

9.I/WeimplementedBactrocera dorsalis surveillance trapping in compliance with the relevant guidelines as provided by DAFF and the surveillance report are attached to this document (available on DAFF website),and accept that ongoing compliance is a pre-requisite for subsequent registration for export to these markets.

10.I/We accept the decisions, protocol, Good Agricultural Practices (GAPs), Standard Operating Procedures (SOPs) inspections and requirements of the South African National Plant Protection Organisation (DAFF: DPH & DIS) as final and in accordance with the applicable export programme (s).

I herewith, in my capacity as producer, declare that I understand all the registration guidelines and take full responsibility for, and comply with, all the requirements as prescribed in the respective export protocols for the export of fresh fruit to the country / countries as indicated under Sections A and B.
