Adoption Agreement

I, the undersigned Adopter, do hereby agree to abide by the following terms, as I adopt my pet from PAWS. My adoption donation is for the furtherance of humane work by PAWS. This adoption donation is in no sense to be regarded as a price paid for the animal, which is given to my custody.

Please read and initial each statement:

Upon adopting this pet, I agree to provide humane care (proper food, water, shelter and exercise); see that the pet is given regular preventative care, vaccinations and immediate medical treatment by a licensed veterinarian if it becomes ill or injured; not permit the animal to be used for vivisection, experimentation or other laboratory work; accept this animal as a household companion and keep it as such; comply with all applicable state, county and municipal laws and have the pet properly licensed.

Once I leave PAWS, I accept all legal responsibility for this animal.

I understand that although PAWS has given initial medical treatment to provide temporary protection while in their care, this animal may be ill or become ill, or may have or develop behavioral problems, and I understand that PAWS cannot guarantee the health or behavior of this animal. PAWS is also not responsible for any resulting illness, injury or damage to any other pets in my home or elsewhere that result from this animal.

I understand that all expenses (including those associated with health or behavioral problems) incurred after adoption are my responsibility, and PAWS will not reimburse me for these expenses.

I hereby agree to hold PAWS harmless and its agents, officers, directors and volunteers, or any of their dependents, from and against any and all claims, damages or causes of action which arise in any way related to the pet I am adopting pursuant to this Adoption Agreement, including the expense of reasonable attorney's fees.

I agree that if I decide the animal does not fit into my household after a reasonable amount of time, and if I am unable to find an appropriate home for the animal myself, that I will contact PAWS and enlist their assistance in finding a new home for this pet.

I enter into this agreement with PAWS in good faith. I have read and understand it completely.
