  • Looking at TOR, Governance structure and CCP.
  • Looking for a co-chair
  • Amanda Trunley will be attending the Community and Primary Health Strategic Advisory Committee meeting to be held on 28th April as the Collaborative representative.
  • Still looking at how this committee will work, escalation pathway for issues.
  • Work in progress

4.2 / Care Coordination Panel
  • Maximum of 3 referrals per panel.
  • Referrals received have been good and quite complex.
  • Good representation from all areas and great to have housing representatives attend.
  • Currently more referrals from the Logan area have been received.
  • Dual Disability a big issue re sustaining tenancies.
  • Would like referrals to be received at minimum one week prior to panel.
  • Referrals are sent to and reviewed by the SIC’s who will then invite the specific agencies to be present.
  • QPASTT would like to be invited if refugee or CALD consumer is referred.
  • Referrals to be discussed are sent out with the agenda prior to the meeting so agencies can review them.
  • Discussed holding 3 monthly reviews of previous referrals to follow up on outcomes and gather data.
  • Feedback from attendees has been good.
  • Next meeting to be held on 2nd May, Building 19 GCOP, 9.30 – 11.30 am.
  • Common theme is lack of funding for supports with ADL’s.
  • If you are not on the CCP email list and would like to be please email .

4.3 / GSJMHC website and newsletter

4.4 / NDIS update – What is your agency doing in response to NDIS
  • There is a lot of uncertainty and a number of agencies are doing their own things.
  • Beaudesert are currently in transition.
  • MH Branch are in discussion with NDIA
  • Townsville – learning a lot re plans.
  • What client has to think about prior to completing plans
  • If there are a number of people at the same address and sharing services – cannot have plans for the same thing – this is causing a number of issues.
  • Families can start plan prior to going to NDIS.
  • Disability Service workers are discouraged from attending planning appts.
  • It is really important to look at who is supporting the consumer and what are the costs of peripheral supports.
  • MH Branch has advised language and terms used is important e.g. ‘Life Event Change’ when being discharged from Inpatient Units.
  • Plan should be done on what is the client’s worst day.
  • NDIA have targets and will want to do plan over the phone, client can say No, I want to do in person.
  • You can download a plan and start completing from the People with Disabilities website.
  • Now is the time to support our clients to get a good plan and to ensure all issues are fully considered.
  • Plan can be reviewed; client does not have to accept the initial plan.
  • Trying to change the plan after accepting the initial one is a very time consuming process.
  • MSAMHS are reviewing consumers who may be covered by NDIS and ensuring they are linked with NGO’s.
  • Siblings Australia currently run a support group at Toowong, if you would like more information visit the website
  • Carers Qld hold the local Coordinator role in Beenleigh.
  • Be sure to include transport costs.
  • NDIA do not want to provide for transport costs where public transport is available so make sure you clearly document why public transport cannot be accessed.
  • CALD – Interpreting Services are not part of packages, currently provided by Harmony Place but NDIS only account for support worker services.
  • How do they organise and pay for interpreter services?
