CLINICAL STAFF:

Dr Divanka Wijendra will be finishing at the end of January 2016 and Dr Emma Stringer will be starting in February 2016. We have welcomed Dr Aswin Thampy (GP) to the practice and have said goodbye to Dr Al-Allak.The practice is in the process of trying to recruit to the clinical team

UNIVERSITY STUDENTS:

Meningitis ACWY vaccination is still available for students home from university for Christmas, who have notyet received it. This is available to anyone age 18 years on 31st August 2015. Please book an appointment with a Practice Nurse

FLU CAMPAIGN 2015:

Have you had your flu vaccine yet? If you are in any of the high risk categoriesand have received an invitation to attend, there is still time to have your annual vaccination.

Please ring the surgery and arrange an appointment for vaccination

HEALTH CHECKS FOR 40 YEAR OLDS AND OVER:

If you are 40 years or over and have no pre-existing significant medical illnesses, please contact the surgery to book an appointment with Sue, our Assistant Practitioner. These checks are used to help promote good health and also to screen you for problems such as high blood pressure, risk of heart disease/stroke and diabetes, so that these problems can be diagnosed early and treated more effectively

CQC VISIT:

The practice had a CQC inspection on 8 October 2015. We would like to thank all our patients who participated and contributed to the inspection. Overall, the practice was rated as GOOD. The results of the inspection are available via the practice website

ON LINE SERVICES:

We offer patients the opportunity to book, check and cancel GP appointments and order repeat medications 24 hours a day, 7 days a week.

To register for this service, you will need a completed consent form (available at Reception),

1 form of photographic identification (Passport or Driving Licence) or 2 forms of any other non-photographic identification (eg utility bill and bank statement) to the practice.It is most important that you thoroughly read the information on the consent form, prior to completing.

The Receptionist will then issue you with a unique username and password. You will be given a print-out of your username, password and the practice website address

There is also an Online App available- ask at Reception for details.

FRIENDS AND FAMILY:

The Friends and Family Test is a patient feedback survey that enables the patient to inform us of their opinion with regard to service and treatment. It consists of just 2 questions.

There are forms in the wall tray, next to the main entrance. Once you have completed the survey, please hand the completed form to the Reception desk. This survey can also be taken on line at

SELF CHECK IN:

If you have made a routine appointment (non-urgent) with anyone at the surgery, you can check in for your appointment via this screen using the following easy steps: Click Start, Press the month you were born, press the date you were born, Press the 1st letter of your surname and you should now be checked in as arrived.

AUTOMATED TELEPHONE BOOKING SERVICE:

Routine appointments for a GP can be booked 24/7 using the automated booking service. Telephone the practice on 01204 462200 and select Option 1.

We must have an up to date telephone number for you, to enable this service.

CORRECT CONTACT DETAILS:

If you have changed your address, home telephone number, mobile number, or email address, please inform Reception.
