GENERAL DOCUMENT REQUIREMENTS

1.Collect the documents described below and review them carefully.

2.Be sure that all required documents are included, clearly labeled, legible and organized. Missing, illegible or disorganized documents will cause delays.

3.For any document not in English, include an official, verbatim, English translation.

CHECKLIST OF ITEMS TO RETURN TO THE DEPARTMENT

Please enclose all items in the same package, organized in the requested order. Do not send documents to the department one at a time.

a.Complete all sections of the Web Form, Part II, online.

b.Print only the signature page and sign in BLUE (not black) ink.

c.AFTER you have printed and signed the hard copy, submit the Web Form, Part II, online.

___ 3.Current, updated Curriculum Vitae (CV) or Résumé.

___ 4.One (1) copy of the biographical page of your passport showing your photo, name, date of birth and the expiration date.

___ 5.Original financial documentation. See the “Funding Requirements” information below.

STEPS TO TAKE

  • Review all documents carefully to be sure that everything is included, clearly labeled, legible, and organized. Missing, illegible or disorganized documentation will cause delays.
  • Assemble the documents in the order indicated in items 1-6 above.
  • Place this checklist on top, then staple or clip all items together.
  • Submit all hard copy materials to your designated department contact. Be sure this Checklist is on top of the documents. Be sure that you have signed the Web Form Signature Page in blue ink; black ink cannot be distinguished from photocopied text. Be sure that you have English translations if required.

FUNDING REQUIREMENTS

SECTION I: MINIMUM FUNDING REQUIREMENTS

You are required to show funding at a rate of $2,107 per month.

This is an absolute minimum calculated from living expenses for a frugal graduate student in the Duke/Durham area, and does not allow for luxuries or emergencies.

Your Duke Internship can be designed with or without wages or other compensation.

If your internship will include any type of compensation from Duke, your appointment letter will contain the hourly or monthly wage that you will be paid at Duke.If your wage/salary at Duke will meet the $2,107/month minimum for your entire stay in the U.S. under Duke’s intern program then you are NOT required to show additional funding from other sources.

If you will not receive compensation from Duke, work full-time or work every week, or if there are other reasons that your Duke wage/salary will not meet the minimum, then you must show funding from others sources to cover your stay here.

SECTION II: ACCEPTABLE PROOF OF FINANCIAL SUPPORT

We must have appropriate proof of financial support before we issue the DS-2019.

You are permitted to show funding from a single source or from a combination of sources to meet the required minimum. Sources of funding might include:

Duke funds. Your appointment letter will show your work arrangements and wage/salary.

Personal or family funds. If you will be funded by personal or family funds, present current bank statements or bank letters showing personal savings sufficient to meet the minimum funding requirements. These documents must state a specific amount of funds you have available to you. If the documents you submit are in a family member’s name (e.g. a parent, guardian, spouse) they must be accompanied by a signed statement from that person confirming the following:

  • the relationship of that person to you,
  • the intent of that person to sponsor your stay in the U.S.,
  • the amount and currency,
  • the duration of their support, and
  • if they are currently in the U.S., their citizenship and U.S. immigration status.

Funding from a foreign government or foreign employer or other non-Duke funding.

If your funding comes from sources other than personal or family funds, present an original, official letter on letterhead from the funding source (i.e. employer, funding organization, university, etc.) confirming the following three items:

  • the total amount of funding to be provided to you, - specify the amount and currency,
  • the dates during which funding will be provided, and
  • the signature of a person in the organization who is authorized to guarantee those funds.

All documents not in English must be accompanied by an official, verbatim, English translation.
