Instructions for Parents registering a Girl Scout – Please complete all sections

  1. a. Check “Reregistering” or “New Registration” at the top of the form
  2. Complete girl’s name, address, telephone number, and the family email address (not the girl’s personal email address)
  3. Her date of birth
  4. Her grade in school for the membership year running from October 2009 – September 2010
  5. School name (the school the girl will be attending for the 2009-2010 school year)
  6. Number of years in Girl Scouting
  7. She is under the custodial care of (check one circle)
  1. Complete the mother/father/guardian information
  2. Complete the emergency contact information (can be a parent or guardian)
  3. Sign and date the permission area (one or both parents/guardians). Note: photo permission is included. Please cross out lines and write on form if you deny photo permission
  4. Complete the racial/ethnic background area
  5. Indicate the amount being donated to the Annual Fund ($40 is recommended minimum donation per family)
  6. Attach a check payable to Girl Scouts of Greater Atlanta for both the $12 membership fee and the Annual Fund donation or complete credit card information if paying by credit card
  7. Attach a check payable to South Forsyth Service Unit for the $3 service unit fee
  8. If you have daughters in other troops, please indicate what troop(s) they are in by writing it in the blank portion of the right hand column
  9. Please indicate the total amount paid for both Registration and the Annual Fund in the right hand column (Do not include the $3 Service Unit fee in this total)

Instructions for registering as an Adult Member – Please complete all sections

  1. Check “Reregistering” or “New Registration” at the top of the form
  2. List all position code(s) and troop number(s) that you will be working with
  3. See GSUSA position codes in the right hand column
  1. Complete the name and address area
  2. Complete name, address and telephone number information
  3. Please provide your email address (this is our primary means of contact and distributing information)
  4. Please provide your employer’s name
  5. Please provide your occupation
  6. Please provide your work phone number
  7. Please indicate the number of years you have been a Girl Scout both as a girl and as an adult
  8. Sign and date the photo release area
  9. Optional area where you can join as a lifetime member rather than as an annual member
  10. Complete the racial/ethnic background data
  11. Indicate whether you are male or female, your age range, and education level
  12. Optional section for remitting a contribution to the Annual Fund
  13. Attach a check payable to the Girl Scouts of Greater Atlanta for both the $12 membership fee and the Annual Fund donation or complete credit card information if paying by credit card. (Adults do not need to pay the $3 Service Unit fee)
  14. Please indicate the total amount paid for both Registration and the Annual Fund in the right hand column

Instructions for Troop Leader

  1. Review all girl and adult registrations to make sure that all information has been completed
  2. Check that at least one parent has signed the girl membership form
  3. Check that all adults have signed their registration forms
  4. It is necessary for United Way funding to record the ethnic/racial statistics, so if the parent has not completed the racial and/or ethnic background, please complete it to the best of your knowledge/observation and:
  5. In the right hand column check “VS Category #1” if you completed the racial portion
  6. In the right hand column check “VS Category #2” if you completed the ethnic portion
  7. If the parent already completed the ethnic/racial section, leave VS Category #1 and #2 blank
  8. Please ensure that the check for the $12 membership fund and Annual fund donation are made payable to Girl Scouts of Greater Atlanta
  9. Please ensure that a $3 check for each girl is included and made payable to the South Forsyth Service Unit
  10. Fees for adult, girl and/or sibling registrations may be paid with one check as long as they are submitted with the same troop. If they are submitted with different troops, separate checks are required.
  11. If the parent has not indicated sibling’s troop on the right hand portion of the girl form, please do so
  12. Record the code numbers in the bottom section of the right hand column
  13. Council code: 329

  16. Registration Area: 101
  17. Complete a Membership Dues Summary Form for each batch of forms submitted
  18. Indicate ending membership year (for the 2009/2010 membership year it will be 2010)
  19. If you are registering for a troop, check the circle next to “Troop”
  20. Select the appropriate program duration (in most cases it will be 12 months)
  21. Select meeting frequency
  22. Select appropriate age level for the majority of the girls registering at this time
  23. Select type of meeting place
  24. Record specifics regarding meeting day, time and location for troop meetings
  25. Enter:

i)Number of girl registrations attached

ii)Number of adult registrations attached

iii)Total number of girl and adult registrations at $12 each

iv)Total dollar amount of membership dues attached (number of registrations X $12)

v)Total dollar amount of contributions to Annual Fund

vi)Grand total of dollar amount submitted (not including Service Unit fee)

  1. Complete Name and address information for person submitting forms
  1. Enclose all copies of membership forms and dues summary form in envelope provided with all payments
  2. If any fees were paid with cash, please deposit all cash into your troop account and write one check payable to either Girl Scouts of Greater Atlanta (dues and Annual Fund) and/or to South Forsyth Service Unit (service unit fees). Please note on the appropriate forms which ones were paid in cash. Cash payments for existing troops will not be accepted by the SU.
  3. If you wish to retain a copy of the registration forms for your records, you will need to make a copy of each form
  4. Please note that the health record is no longer on the back of the registration form, so you will need to have separate health record forms completed for each girl. You can find those forms on the Girl Scouts of Greater Atlanta web site
  5. Return packet to Brenda Mullins, the Service Unit Registrar, at the following address:

  1. You may also submit the packet to Brenda Mullins at the next SU meeting
  2. Do Not mail or submit packet to a council office. Your troop’s information will not get recorded in the SU database (used for membership verification for product sales and events) if you submit the packet directly to the council.
