UNM IM daily triage packet

Inpatient admission/transfer process – data collection

The UNM IM Program Evaluation Committee is studying our inpatient admission system in order to optimize work flow and increase learning opportunities for our residents. We are using the daily triage sheets to analyze the admission and transfer processes. We appreciate your help filling out these triage forms as completely as possible.

One triage packet will be used per day, 7 am – 7 am.

Monday through Friday

1)7 am: Silver starts a new packet (sheets from prior packet with patients still to be seen will be attached to the new packet)

2)Noon: Silver hands off packet to day call team

3)7 pm: day call team hands off packet to night float team

4)7 am: Adriana will pick up packet from night float team in Jemez Room

Weekends

1)7 am: day call team starts a new packet (the Day Call attending will provide the new packet to the Day Call Resident – (sheets from prior packet with patients still to be seen will be attached to the new packet).

2)7 pm: day call team hands off packet to night float team

3)7 am: Night Call attending will pick up packet from night float team in Jemez Room – (sheets from prior packet with patients still to be seen will be attached to the new packet).

MICU accept (This procedure is unchanged during this study period – the MOT team will be using the old template.)

1)Print of triage sheet from UNM hospitalist wiki

2)Record patients evaluated for transfer on triage sheets

3)Turn in triage sheets to MICU triage sheet box in each physician workroom

Miscellaneous admission/transfer, bounce backs

If you end up doing a miscellaneous admission or transfer, please use a triage sheet on the Wiki (as is being used by MOT) to record the patient and put the sheet in the MICU triage box per above.
