I Understand That My Taxable Salary Will Reduce by the Amount of Any Salary Sacrifice (Pre-Tax)

This form is to be used by employees to change your rate of voluntary and / or spouse contributions, or to change your “type” of voluntary contributions.
Action code and contribution rate fields are mandatory and have to be updated for successful completion of the request.
Employees can contribute maximum of 90% of their salary towards Superannuation contribution as per the ATO requirements.
Complete all sections of the form and save and email to.
If you are not currently making any voluntary contributions, you should submit your request online using Employee Self Service > My Superannuation on Max.

Eff date of change / This contribution change will be effective from the start of the pay period commencing after your form is received by People Assist.
To comply with the Australian Taxation Office (ATO) requirements, the start of the pay period from which changes to voluntary pre tax contributions are effective must be after the request is submitted.

  • I understand that my taxable salary will reduce by the amount of any salary sacrifice (pre-tax) contributions.
  • I understand that contributions tax of 15% will be deducted from these contributions when they are paid to my super fund and additional 30% tax plus Medicare levy will apply to all contributions in excess of the concessional contribution limit or if I have not provided my Tax File Number (TFN) to my super fund.
  • I understand that the onus is on me to check that I don’t exceed the concessional contribution limit and penalty tax will apply if I do.
  • I understand that salary sacrifice contribution elections must be made prior to income being earned, and Payroll will process this election in line with the ATO requirements.

  • I understand this option is available only if I have provided my TFN to my super fund, otherwise these contributions will not be accepted.
  • Any post-tax contributions in excess of the applicable annual or three year limits will be taxed at a rate of 45% plus Medicare levy.

  • You may only add a spouse contribution if your spouse already has a Spouse Contribution Account in the ANZ Australian Staff Superannuation Scheme. To set up an account, complete the application form in the Spouse Contribution Account PDS on the Forms and Publications tab.

  • TEC Managers: 9% of your Notional Salary will be debited from your TEC remuneration package to provide Group Superannuation Support.
  • Voluntary contributions may be made on a:
  • pre-tax basis, which count as Concessional contributions and will be classified as Reportable Employer Superannuation Contributions (RESC) ; or
  • After-tax basis, which count as non-Concessional contributions.
  • Limits apply to the amount of Concessional and non-Concessional contributions you are able to make on a Concessionally taxed basis. If you exceed these limits, penalty tax applies.
  • These limits are available on an Educational Facts Sheet entitled “Superannuation Facts and Figures” which is published on under the Forms and Publications tab.
