I am pleased to offer you employment within the McGoogan Library of Medicine at the University of Nebraska Medical Center (UNMC). Enclosed you will find the UNMC Library of Medicine Employment Agreement, including Appendix A (Duties and Responsibilities). To confirm your acceptance of this offer, please sign and initial (at the bottom of each page) the enclosed documents where indicated and return to Ann Newman, Library Administrator, in the envelope provided.

In order to process your employment paperwork and achieve the intended start date, it is essential that you return the signed documents by (date). Please note – You will not begin your employment with UNMC if we have not received the signed documents by this date. In addition, as referenced in your Employment Agreement, your appointment is contingent upon completion of an acceptable background check. You will receive an e-mail from a company named One Source, which will contain a user name and password for you to access the One Source secure website and complete a short on-line information and authorization form. To verify your credentials, please send the official certified transcript for your degree to Ann Newman.

To comply with the immigration Reform and Control Act of 1986, UNMC must verify the identity and employment eligibility of all new hires by completing Form I-9. Attached is a Form I-9 for your review. Please bring the applicable documents that establish both identity and employment eligibility (refer to List of Acceptable Documents on the back of the Form I-9).
