HRC Contract Variation Request Form

This form is used to provide information to the HRC about proposed variations to current research contracts with the HRC. Attach other documents or evidence as necessary. The HRCmay request further information in order to determine whether or not the request should be approved.

Note: Host organisations must notify the HRC as early as possible that they intend to submit a variation request. The HRC will make the final decision as to whether or not the variation is approved. The variation will not be considered as in effect until the HRC has notified the Host organisation that approval has been granted.

Note: If this contract is funded through the HRC’s Partnership Programme, please confirm that this request will not impact on the contract meeting any of the requirements as outlined in the original Request for Proposals (RFP) document (see Section G).

If this contract is funded through the HRC’s Partnership Programme, please confirm that this request will not impact on the contract meeting any of the requirements as outlined in the original Request for Proposals (RFP) document.

Notes

1)Variation requests should be sent as soon as research performance issues arise, not within the last month of the contract.

2)Research progress reports should alsodocument delays or significant issues that have affected the contract.

3)Work that extends or significantly goes beyond the research originally proposed may be outside the scope of the contract, so a contract variation would not be appropriate.
