Guide for Completing the Electronic Individual Survey

It is very important that you complete the Communal Discernment Process in a small group prior to completing the Individual Survey.
The following link can be used to access the electronic Individual Survey:
The Individual Survey could take up to one hour to complete.
The link to the electronic Individual Survey can be accessed on the internal portion of the SCN website by clicking on the Government Committee Tab.

You will need to select one of the options to each question(s) in each Step before proceeding to the next Step.
The “Why” part of each question is optional but encouraged as your written rationale will give the Government Committee a better understanding of the option that you chose.
Responses to the “Why” part should be brief. It may help you to draft your written response to the “Why” part prior to entering it electronically.
You do not have to complete the Individual Survey in one sitting. You can save the completed portion by clicking on “Save and Continue Later.” When you click “Save and Continue Later” you will be asked to enter your email address to which a link to your saved Individual Survey will be sent. The email will contain the following message: “Thank you for saving SCN Plan of Government Survey. Please use the unique link below to return to the form from any computer. [link will be here] Remember that the link will expire after 30 days so please return via the provided link to complete your form submission.”
When you want to continue the Individual Survey, simply click on the link in your email and it will take you back to your responses.
When you complete the entire Individual Survey, make sure that you click the Submit command and your responses will go directly to Spalding Hurst at Nazareth, KY.
Do not click Submitunless you are sure you want to submit your Individual Survey responses. Once you click Submit, you cannot go back to edit your responses. If you feel your responses need to be modified, you cancomplete another Individual Survey (just inform your Province CommunicationsCoordinator so your first responses can be deleted).
