Scholarship Application

Females About Business (FAB) will administer five (5) $500.00 scholarships funded by proceeds from the FAB Gala. This scholarship will be offered in collaboration withThe DeLand Area Chamber of Commerce professional networking and development group, Females About Business (FAB), to encourage and promote the professional development of women throughout West Volusia to achieve greater levels of success both personally and professionally and to serve as examples to their peers in doing so.

In general, a successful applicant must demonstrate willingness to inspire other females in business and show a keen interest in her professional development by participating in organizations, holding leadership positions and participating in activities that benefit the community either civically or philanthropically. She is a problem solver, a committed doer, and an effective communicator. She should be an example to other women as a potential leader, both professionally and socially.

Additionally, applicants need to meet the following criteria:

1. Applicant must have an active paid membership in the DeLand Area Chamber of Commerce.
2. Be in a position to complete the attendance and other commitment guidelines associated with education opportunity selected.
3. Applicant must be active participant in Females About Business (4 or more events)
4. Supply all the information required for a complete application.
5. Meet the application deadline.

If you meet all these qualifications, please complete the enclosed application according to the following instructions. The timeline of the application process is as follows:

(1)Completed applications must be postmarked byMonday, December 1, 2014.

(2)Applications will be reviewed and scoredduring the month of December 2014.

(3)Award notification letter will be sent no later thanJanuary 1, 2015 and will inform the recipient of the scholarship award. Each$500 scholarship will be distributed afterthe applicant receives the acceptance letter from the proposed institution or organization conducting the continuing education or training.

(4)The scholarship recipients will be honored at the first FAB quarterly luncheon to fall after the education acceptance letter is received.

INSTRUCTIONS FOR APPLICANTS

To be accepted for review, an applicant must submit all required items and have them postmarked no later thanDecember 1, 2014. These items can either be submitted all in one package (mailed by the applicant herself) or separately (each person serving as a reference mailing their own letters directly to the address below). Be sure to fill out the names of the references on the application, especially if mailing items separately. The applicant is responsible for ensuring that all necessary information (completed application form, resume, applicant essay, and letter(s) of reference) is submitted by the deadline. The applicant will be disqualified from review, with no further follow-up, if these instructions are not followed.

APPLICATION FORMS: Official scholarship application forms are made available in person at the DeLand Area Chamber Office or they may be requested directly from the DeLand Area Chamber of Commerce "Scholarship Application Request to use of copies or reproductions is permitted.

MAILING INSTRUCTIONS: Mail all items by regular mail postmarked byDecember 1, 2014 to the Scholarship Chair (address shown below). Please do not send registered or certified mail.

THE FOLLOWING ITEMS MUST BE SUBMITTED FOR ALL APPLICANTS:

1. COMPLETED APPLICATION FORM:Additional information may be attached to the application if there is not enough room in the space provided.

2.RESUME: A complete resume reflecting the past ten years of experience (or less when applicable). The applicant may determine format of the resume. While many industries defer to a CV (Curriculum Vitae), for the purposes of this application, a resume is required.

3. APPLICANT ESSAY:The essay is limited to 500 words (12 pt. font) in typed, essay format and should describe the applicant's feelings on the following:

Why she feels she is the best applicant to receive the scholarship and why she has chosen the program of continuing education or professional development, including what she hopes toachieve as a result of that choice.

4. REFERENCE LETTER NO. 1**:Letter of reference from a colleague either within your organization or someone you interact with throughout the course of business on a consistent basis. Letter must be dated in the application year.

5. REFERENCE LETTER NO. 2**:Letter of reference from an individual who the applicant identifies as a mentor either formally or informally. This reference can be the applicant’s supervisor. Letter must be dated in the application year.

** Copies of recommendation letters submitted for other scholarships are acceptable, as long as they are up-to-date and reflect the student’s current academic achievement and professional development.
