Supported Independent Living Facilitator

Exciting opportunity exists for the right individual to work in our Burnaby Head Office and in community as a Supported Independent Living (SIL) Facilitator. The successful candidate will be joining a large progressive non-profit organization with growing opportunities. We work with Adults with various disabilities to provide them with resources to live healthy and fulfilling lives at home and in the community.
Work collaboratively with the department Director or Program Manager to oversee, implement, and maintain the program in a fun and flexible environment.
Coordinate and manage a case load of assigned responsibility. Act as a resource to individuals served, their respective family members or representatives and the care providers/support staff. Perform a variety of professional activities in support of the Program including but not limited to administration, home visits, contract enforcement, inspections, placements, advocacy, special projects and program development.
Regular schedule: Monday to Friday 8:30am to 5pm (unpaid lunch) 40 hours per week
·  Must be able to travel throughout lower mainland and Fraser Valley
·  Position includes paid rotating on-call duties
Job Qualifications Include:
·  Minimum Bachelor of Social Work or other Social Science
·  or Social Services Diploma plus experience in the social services field
·  Minimum of One (1) year of case management experience
·  Minimum of One (1) year experience with individuals with disabilities
·  Fluent in ASL
Job Requirements Include:
·  Criminal record check
·  TB test
·  First Aid and CPR
·  Minimum of three (3) contactable references
·  Valid BCDL and Reliable vehicle to use for work purposes
Additional Requirements:
·  Experience with advocacy
·  Experience in the public or non-profit sector
·  Experience working with individuals with physical, developmental disabilities, brain injuries, and/or complex health issues
·  Experience in the social services field
·  Experience working in a team environment with little supervision
·  Experience working with a case management team
·  Demonstrated ability to deal with crisis and emergency situations
·  Computer literacy and experience with databases
·  Excellent written and verbal communication skills
·  Excellent time management skills
·  Knowledge of community resources
Start Date: ASAP
Compensation starting at $20.38/hr and full benefits upon meeting eligibility requirements after three months.
