PTHP Components Checklist
1)PTHP form RM-64:
2)PTHP Checklist
3)PTHP maps
4)Notice of Intent to Harvest Timber
5)Domestic water supply proof of having published notice and written to property owners within 1000 feet downstream, if required (14 CCR §1032.10)
6)Plan submitter agreements and authorizations required under 14 CCR § 1092.11 (d), using the acknowledgment forms available here:
RPF_PS_TLO_LTO_AcknowledgementForm_011310v2.pdf
7)Explanation and justification for exceptions and alternatives
8)EHR worksheet (see checklist item 22)
9)Addendums
a)Biological addendum
i)NSO survey information [If surveys have not been completed before plan submission, provide a map of nesting and roosting habitat within 0.7 miles of the plan, and any activity centers within that range previously recorded in the NSO database.]
ii)Botanical survey [If this is not available yet, identify the habitats and species that the survey will focus on, and state that the survey will be completed prior to operations.]
iii)Other surveys as needed
b)Inventory sampling addendum and silviculture by units (see checklist items 5 & 6)
c)Geology report (if required based on checklist items 17 and 41)
d)Sediment Prevention Plan (SPP) if required based on checklist items 23, 24, 26, 33, or 37
e)Erosion Control Plan (ECP) —see checklist item 44
f)Watercourse
i)Class I restoration [checklist item 63, 14CCR § 916.4(a)(2)]
ii)Class II standard/large determination (checklist item 50)
iii)Spawning and rearing habitat map [checklist item 63, 14CCR § 916.4(a)(2)]
g)Rate of Harvest (see checklist item 71) –MRC or BBW can provide this.
h)Confidential Archaeology
i)Greenhouse Gas statement. No additional calculations are required, but the PTHP should include an addendum that states:
"Timber operationsproposed under this PTHP are consistent with the silvicultural and timber harvest practices outlined in the Mattole Forest Futures Project PTEIR, Section 2.1, Proposed Project, and, as such, are within the scope of the environmental analysis contained inPTEIRSection 3.6, Impacts of Program/Alternative Implementation on Greenhouse Gases. Because the timber operations proposed under this PTHP are within the scope of the environmental analysis contained in the PTEIR,their effect on greenhousegases should be less than significant, as demonstrated inthe PTEIR."
10)Flow Charts (Indicate on maps how the flowcharts were applied, if they were required because of mapped geologic features, landslide hazard rating, or observed geomorphic indicators of instability. Checklist items 16, 27, 29, and 40)
a)Silviculture Flow Chart
b)Yarding Flow Chart
c)Road Flow Chart
Items that do not need to be included because they’re already covered in the PTEIR:
1)Cumulative Impact Assessment
2)Green House Gas Calculations
3)General description of site like 14 CCR § 1034 jj
4)Identification of hazard reduction areas
