Hospital Project Charter

Project Dates: January 2012-December 2014

As the IHC Hospital Engagement Network (HEN) is launched, it is important to remember the pursuit of the Partnership for Patients campaign goals. The Partnership set two-crisp aims:

  • decrease preventable hospital-acquired conditions by 40% by the end of 2013
  • reduce avoidable hospital readmissions by 20% by the end of 2013.

The HEN is not a project designed to change the work currently being executed in hospitals. It issimply an effort to provide assistance in improvement initiatives and the spread of best practices across a high-quality performing state.

This initiativeidentifies 10 focus areas that will affect the future quality and patient safety of our healthcare system: adverse drug events, catheter-associated urinary tract infections, central line-associated blood stream infections, injuries from falls, adverse obstetrical events, pressure ulcers, surgical site infections, ventilator-associated pneumonia, venous thromboembolism, and preventable hospital readmissions. Each hospital will participate in all applicable focus areas, as well as Lean and culture of safety organizational initiatives. The HEN is a three year opportunity, with current funding provided for the first two years.

Services available to hospitals in the Partnership for Patients Hospital Engagement Network:

  • An Improvement Advisor assigned to each hospital to facilitate work plan development and execution
  • Access to face-to-face Learning Communities using the IHI Breakthrough Series Model throughout the year to share and spread best practices
  • Topically-focused webinars, conference calls, and toolkitspresenting latest content
  • A web-based quality/safety measurement reporting system (QMRS) developed by IHA to provide real-time information to lead process improvement efforts
  • Leadership resources to assess organizational readiness, leadership engagement, and sustainability
  • Topical learning networks that will allow hospitals to collaborate in an effort to raise the standard of healthcare
  • Lean educational trainings and on-site coaching
  • Available onsite technical assistance provided by Telligen to move internal process improvement work where needed
  • Physician engagement strategies

Expectations of hospitals participating:

  • Identify patient safety and quality teams to lead local improvement efforts
  • Appointhospital employees to serve as contactsfor HEN related activities
  • Work with IHC Improvement Advisors to capture current interventions and work plans on applicable focus areas at your institution and develop a work plan in alignment with the national aims
  • Report selected process and outcome metrics monthlyfor applicable focus areas via a web-based data system
  • Develop reporting capabilities with the CDC’s National Healthcare Safety Network (NHSN) and grant IHC shared access to the data
  • Participate in IHC HEN Learning Communities
  • Complete an Organizational Assessment Tool (OAT) annually to measure current performance
  • Complete a Leadership survey annually
  • Promote physician engagement with the IHC Medical Home initiatives
  • Pursue a culture of safety and accountability
  • Engage executive management, medical staff, and boards in HEN activities
  • Position the hospital to lead community discussions on care coordination and transitions to reduce readmissions
