Subject: Customers’ Data Review
In order to comply with the requirements of the Central Bank of Cyprus Directive on the Prevention of Money Laundering and Terrorist Financing, AstroBank Limited (“the Bank”) is required to review and update its records of information for all its customers.
To ensure that the Bank’s records are up-to-date, you are kindly requested to complete the attached forms with your personal information and submit them together with the following documents:
1. Copy of a valid ID for citizens of the Republic of Cyprus or a valid passport for citizens of other countries
2. Copy of a valid and recent (up to 6 months) utility bill such as electricity, water or land line telephone bill. The utility bill shall be in the name of the account holder and/or joint account holder and include the permanent residential address of the account holder.
3. Copy of a monthly payslip.
Once the information obtained is assessed, the Bank will notify you if any additional documentation and/or information is required.
The completed forms and documents can be sent to the Bank as follows:

In the event that you fail and/or deny and/or neglect to provide the above-mentioned requested information, the Bank reserves the right to proceed, depending on each case and at its absolute discretion, with blocking or closure of the account(s) and/or termination of the relationship (until this is fully regularized pursuant to the provisions of this letter).
