Badge Reprint Fee: The AAO will allow one free re-print onsite, then $50 per re-print; only one re-print per

day. This applies to ALL – doctors, staff, spouse/guest, exhibitors

What are the guidelines for Cancellations / Refunds?

Requests for registration cancellation / refunds must be received in writing, including name and registration/badge number, and sent to the attention of the Sarah Dvorak via e-mail ,

fax 800.747.2513 or mail (401 N. Lindbergh Blvd, St. Louis, MO 63141).

Requests received after June 2, 2016 (30 days after meeting) will not be approved.
