The IACUC must review and approve the use of all compounds before they are used. * There is no need to amend the protocol for changes in the vendor of USP or pharmaceutical grade ingredients. Changes to non-pharmaceutical grade compounds must be first reviewed and approved by the IACUC.

Preparation: Is there further dilution before use? If not, “As above.”

Sterilization: The cocktail is prepared aseptically from sterile ingredients.

Volume administered:1ul of cocktail per gram of body weight.

Labeling:

Per DEA guidelines, the label must contain:

  • Name and volume of each ingredient, including the diluent
  • Lot number of each controlled substance
  • Final concentration of each drug
  • Total amount/volume of the combined drugs
  • Date of preparation
  • Date of expiration*
  • Initials of the individual disbursing the compound
