How to use these forms

These forms have been designed to assist both the new member of staff and their line manager in reviewing performance and monitoring progress against agreed objectives during the crucial first months of employment. In completing these forms the line manager should refer to the Probation Process policy Document.

The line manager will be responsible for ensuring reviews are held and for the completion of these review forms. During the probationary period the forms are held by the line manager. The completed forms should only be sent to Human Resources at the end of the probationary period. Human Resources will write to the individual and their line manager confirming employment once these have been received.

Probationary Review Process

First week - informal review

Provide orientation by going through induction checklist parts A & B and setting initial objectives during informal review meeting. Identify and agree any initial training needs, complete form PR-INF.

First month - interim review

By end of the first month, complete annexes 1 & 2 prior to the review and then carry out one month review using the Interim Review Form (PR-INT) including setting further objectives. Attach annexes 1 & 2 to the Interim Review form.

Third month - final or interim review

Employee should have attended University induction by now

Staff in grades 1-3 – final review - complete annexes 1 & 2 prior to review, carry out final review and build on future performance by agreeing new objectives to be carried forward to the appraisal. Complete Final Review Form (PR-FR), attach annexes 1 & 2 and send all the probation forms to Human Resources who will then confirm employment.

If probation is to be extended or has not been completed satisfactorily please contact Human Resources before conducting review.

Staff in Grades 4-6 – Interim review - an interim review should be held by end of the third month. (see First month above for process).

Sixth month - final review (Staff in Grades 4-6 only)

Complete annexes 1 & 2 prior to review, carry out final review and build on future performance by agreeing new objectives to be carried forward to the appraisal. Complete Final Review Form (PR-FR), attach annexes 1 & 2 and send all the probation forms to Human Resources who will then confirm employment.
