CANCELLATION POLICY: Our contract with the hotel stipulates that the college will be held accountable for cancellations that exceed 20% of the number of rooms requested. Thus, those who cancel early are less likely to forfeit their deposit/private room payment with odds of forfeiture increasing if cancellations approach the 20% figure. Also, cancellations within 24 hours of your arrival date may be subject to a one night’s room charge. However, we will try to locate a replacement participant so that we can fill your room and refund your payment. Van riders can cancel up until May 2, 2011.
WORKSHOP SELECTION FORM FOR SPRING 2011 FACULTY RETREAT
On this form indicate your top five choices (with one being your first choice and five your lowest choice) for workshops at the May 2011 faculty retreat. You will be attending three of your selections. However, we are asking that you choose five to give us some flexibility as workshop space is limited. Also included areseveralleisure time activities. If you are interested in participating in a free time activity, place a check mark by each activity, knowing how many are interested in a leisure time activity will help the presenter prepare for the leisure time activity! Refer to the attachment entitled “2011 Retreat Workshop Descriptions,” for a detailed description of each workshop.
Directions: Number (1-5) your top five choices for retreat workshops. If you want additional information about a particular workshop feel free to contact the workshop facilitator/presenter. Return this form to the Office of Faculty Development no later than April6th. After we have received your choices, we will contact you with names of the workshops you will be attending. Persons facilitating a workshop are asked to include their own workshop among their choices as their number one choice. If we do not receive your workshop selection by April 6th, we will assign you to workshops based on available space.
