HOW TO USE THIS WORKSHEET

This worksheet should be used as a template for you to prepare your application content. Utilize the space below the section headers to complete your responses. You may then simply copy-and-paste your responses into the online application, which you will be able to access starting December 1, 2014.

Remember:

  • Keep your text formatting simple (no italics, bold, underline, etc.), only basic spacing will be saved in the application.
  • Character count includes all spaces and punctuation. Visit for a free character counter.

SECTION I: ARTIST PROFILE & COLLABORATION INFORMATION

You will be required to provide basic information including your name, website, birth date, county of residence and basic questionnaire.

If you are submitting a collaborative application, you will be asked to provide the name, address, and contact information for each of your collaborators.

SECTION II: PROFESSIONAL PRACTICE QUESTIONNAIRE

You will be required to complete a brief questionnaire regarding your interest and access to arts resources in your community.

SECTION III: RESUME[LIMIT: 12,500 characters, approximately 2,000 words]

Your resume should include your education and a selected list of your professional achievements that reflect your artistic accomplishments in the category in which you are applying, such as exhibits, readings, honors, awards, commissions, collections, residencies, lectures, and publications. Format as a list, in reverse chronological order, listing the most recent dates first.

The discipline statement should provide additional explanation of your discipline, describe the specific genre your submitted work samples fit into, and provide any additional context for your discipline (i.e. the audience for your work, or a specific influence).

The artist biography is a concise snapshot of you, your artistic career and practice, written in third person. It may note key themes in your practice, your medium, career highlights or other relevant information. Think of this as your “elevator pitch”.

Clearly explain your artistic work, vision and inspiration. You may want to address what your work means to you, what materials/techniques you use and why, where you find your inspiration to create, what ideas you are trying to communicate, the life experiences that have influenced your artistic vision, how your work relates to your development as an artist and/or how your work relates to other artists' work. The artist statement should be written in first person, present tense.

The community impact statement should explain how your work impacts and inspires individuals and groups around you. It should provide insight into how your work reflects or enhances communities, locally or nationally, such as artistic communities, educational communities, neighborhoods, specific populations or audiences, or those you mentor. It is up to you to consider and define the community or communities that your work impacts. Your statement may speak to the communities you impact currently and/or those you hope to impact in the future.

The narrative statement should explain your overall plans for the fellowship period, how you will use the fellowship time, why the fellowship will be pivotal in the exploration of your art form and what the fellowship will contribute to the advancement of your career. The fellowships are merit-based and are not project grants, however, it is helpful to the panelists to get a sense for how the fellowship will impact your work.
