Job Title:Personal Navigator - Multiple Complex Needs

Department:Housing

Responsible to:Support Team Manager

Location:Based at Shakespeare Street Hostel with travel within the city and county

Hours:37.5 hours per week including evenings and weekends

Salary:£21,000 per annum

Job Purpose

  • Facilitate joint integrated working across the partnership and within the YMCA.
  • Working closely with other team members to ensure delivery is of high quality and meets required standards.
  • Support a defined group of residents who will benefit from a person centred approach to promote independence.
  • The post holder will work with The Support Team, personal budget project and the overall partnership
  • Supporting the assessment and development of an action plan developed jointly with the resident, health and social care professionals based on an assessment of need.
  • Facilitate and support the support team in managing residents with multiple complex needs and as appropriate co ordinate their support and services to be delivered.
  • Using evaluation techniques support the service to monitor the impact of the services on quality of support.
  • To be a point of contact for residents in coordinating their support across the entire partnership and other support provisions within the community.

Principal Responsibilities

  1. Delivering support for those with multiple complex needs
  2. Being aware of national and local policy that impacts upon the health and well being of residents with multiple complex needs.
  3. Working with the multi-disciplinary team promoting independence for residents with multiple complex needs.
  4. Working with residents to plan for and improve their support provisions, ensuring that choices are reflected in personalised action plans and communicate effectively with other involved in their care.
  5. Supporting the team/project to establish effective working relationships with residents. This will include promoting individual rights and recognising and respecting their contributions to support planning and delivery.
  6. Support care act assessments, interpret and discuss assessment outcomes with residents, other professionals and the voluntary sector to agree care plans.
  7. Support residents to be as independent as possible by facilitating a range of self management strategies through undertaking desired activities including the support that is available from other sectors.
  8. Working with partner organisation to coordinate inputs from all other agencies, ensuring that an individual’s support integrated.
  9. Establishing and maintaining excellent communication with individuals and exploring complex issues relating to support options and decisions and sustain effective working relationships across all services.
  10. Working effectively with local health, social care, housing and voluntary sector services.
  11. Collaborating with service providers and residents to develop and review a resident journey.

2 Other Duties

To undertake any other duties and responsibilities as may be assigned by your Line Manager or anyone else designated by the Chief Executive Officer, as necessary. To work in a flexible way to ensure that workload is completed and to undertake any other jobs commensurate with the seniority of the post.

3 Discretion to Act

To exercise discretion in the performance of the duties of the post, to use best practice and to ensure the effective and efficient use of resources.
