  1. What’s good?

Here at Devon Outreach we encourage the people we support to be as active as possible within the community that they live and in the outside community.
We encourage the people we support to try new thingsto promote choice and independence and increasing self esteem and confidence.
Most of the people we support live in a communal Sanctuary extra care supported flats, which have a Social activitiesco-ordinator with daily events and social activities.
  1. What’s not good?

Sometimes due to a large percentage of the people we support being elderly/ having physical limitations, motivating through choice is difficult, For example illness and energy levels being sometimes frail, to go out into the outside community.
One person we support likes to know in advance what is happening and if unforeseen circumstances change plans for example staff sickness, he finds change difficult.
One person we support is not keen to pay for staff supported outside activities. This is limiting his choices as there are only a few places that carers go free.
  1. Whatwe want to happen?

We want the people we support to feel valued and supported in whatever choices that they make.
For there to be the opportunity to maintain and further grow links within the community.
For the time and support to be put in place to see the reasons why the people we support may not want to go out.
  1. What we are worried about?

Lack of communication in the community and the people that we support feeling excluded in some way.
  1. We will do what? With support from? (box 3 + 4)

We will revisit the needs, wants and aspirations of the people that we support using PCP tools.
We will put together a choices file with leaflets, information, pictures of events, courses, clubs and activities etc to help aid us to encourage links to the community.
We will actively look at our rota to see if staff can give more communication support at in house activities, and liaise with the activities co-ordinator.

  1. What’s good?

We have lots of safeguards here at Devon Outreach to help the people we support live a full and active life.
We encourage people to try new challenges, whilst managing risk.
We have risk assessments in place.
We use a PCP matching tool to establish people’s likes and dislikes, goals and aspirations.
We have a pictorial survey each year that the people that we support fill out to help us see what is good and what is not.
  1. What’s not good?

The people that we support want to spend time out in the community and be as independent as possible. There are sometimes barriers to this, as if there is an emergency situation – issues can arise regarding communication.
  1. What we want to happen?

Making an Emergencycard to be carried with contact details on and any other relevant information eg, medical details (Diabetic, Epilepsy)
  1. What we are worried about?

Finding the time to do the above as we are an outreach team and rarely in the office.
  1. We will do what? With support from? (box 3 + 4)

1. We will discuss in our team meeting any ideas to share to move this forward.
2. We will work in a PCP way with the people that we support to ask how they would like the Emergency cards to look and what information they would like included etc.

  1. What’s good?

The people that we support at Devon Outreach always have access to their own money.
The people that we supported who are less able to get out and about independently go with their key workers to ensure they have enough money for the week.
Most of the people we support have a good understanding of their money and they have sole responsibility for it in their own homes.
Most of the people we support have bank cards and pay their bills by Direct Debit/Standing order.
All the people we support have safe guards in place and a good understanding of money security/safety. Keeping pin number private etc.
  1. What’s not good?

Different shops have different amounts to spend on cards eg, Minimum/maximum. Some cards use pin/contactless, You can send cheques via the post but cannot use them in shops for example.
For some people that we support it is a problem as it is so confusing, due to the lack of communication on the shop staff part. E.g. A person we support thinks theyare not allowed in certain shops but it is that the way they are trying to pay is not allowed.
Not being able to understand the ever changing technology that is connected to money transactions and internet shopping.
Struggling to communicate with the shop/ bank staff if they are out in the community independently of support.
.
  1. What we want to happen?

Increase understanding of the different ways bank cards work, new technology for example, contactless payment.
Increase choice and independence for the people that we support when spending their money.
Decrease the stress and frustration that comes with not understanding how the new technologies work.
Make sure that the people we support’s risk assessments are revised in this area to protect them.
  1. What we are worried about?

Possible Fraud or overuse of the bank card if linked to internet shopping.
Misunderstandings regarding online shopping sites eg, Ebay, Amazon and how they work.
  1. We will do what? With support from? (box 3 + 4)

1. Key workers will work in a PCP way spending time discussing with the people that they support, in their preferred communication method, the options available to them and the varied choices that they can make. Allowing them to make an informed choice about payment methods.
2. Risk assessments will be revised and reviewed
3. Using key workers to reinforce the information when out with the people they support in the community shopping/paying bills etc.
4. Fact cards chosen in a format by the people that we support will be made to explain shops minimum and maximum spend etc.
5. Review/revise safety measures ensuring that strict procedure are in place to protect against any possible financial abuse.
