Notes:
· This application is to be filled out by students who are interested in enrolling in one of Athabasca University’s science project courses, i.e. BIOL 495 or 496, CHEM 495 or 496, ENSC 495 or 496, or SCIE 495 or 496. Each course is worth 3 credits.
· Before filling out this form, students should first read the relevant syllabus, detailed course description, and samples of completed projects. Students should ensure that they have completed the appropriate prerequisites. Students must complete the appropriate 495 courses before enrolling in any subsequent 496 courses.
· Before students will be allowed to register in any of these project courses, the appropriate Course Co-ordinator must approve this application. This application may be mailed, faxed or e-mailed to this person. For mailed or faxed applications, please print or type.

E. Time Schedule
1. Expected project start date
· (Usually it takes at least a month between receipt of a completed application and the student enrolling in the course.)
2. Expected project completion date
· Athabasca University allows students six months to complete a 3-credit course. Student loans usually restrict students to four months.
· For these courses, the Learning Contract (see Course Description) usually takes between 4-6 weeks before a student can start work on a project.
· These courses normally take a great deal of time; allow plenty of lead and completion times.
