Clinical Family Services Specialist, Community Casework and Counseling

Provides a professional level of counseling, case management and supportive services to individuals, children, adults, and families. Selects appropriate therapeutic modalities and interventions to assist clients to define problems and formulate and implement treatment goals. Provides services to clients experiencing domestic violence including counseling, referral, and advocacy.

Maternity/Adoption Department/Preservation Worker

Provides a family centered clinical intervention to empower and strengthen relationships in adoptive families, and improve the overall well-being of their members. Qualifying families have adopted children or have children in their care under subsidized guardianship through the Illinois Department of children and educational and mental health advocacy, information and referral, family support groups, and cash assistance in order to maintain the relationships, preserve the adoption, and improve family functioning.

Preferred Requirements: MSW or related. 3 years in child welfare. LCSW or LCPC preferred. MSW or related. No experience.

Case Management/Choices Worker

Older Adult Services, Northeast/Northwest Case Management

Provides direct service to elderly clients needing assessment and casemanagement to remain safely and comfortably in the community. Completes home visits in the community, hospital or in HUD 202 buildings. Conducts assessments, provides information, develops realistic Care Plans, Authorizes governmental services refers and links clients and monitors clients services.
