In consideration of being permitted by Medowie Touch Football Club to participate in the Touch Football Competition agree as follows:
1. To abide by the provisions of any act, regulation or by-law which may have effect upon the competition or by participation therein.
2. That my participation in the competition shall be entirely at my own risk and upon condition that Medowie Rugby Union Club, its servants and agents and each of them shall not be liable in any manner whatsoever to me or any person claiming on my behalf in respect of any act or omission howsoever occurring during or arising out of or incidental to the conduct of the competition or my participation therein and I further agree to indemnify Medowie Rugby Union Club, its servants and agents and each of them against any liability which may be incurred to any other person as a result of or incidental to my participation in the competition.
3. This form must be signed by a parent or guardian if you are under the age of eighteen years of age. (18)
