Financial Policies
Thank you for selecting Sundance Medical Associates for your healthcare services. We would like to inform you of our financial policies.
Insurance & Payments
As a courtesy to you, we will bill your insurance company for any services rendered. In order to do so, you must present your insurance card at the time of your visit.
Without your insurance card, your appointment will be rescheduled.
Co-payments and Deductible payments are always required on the day of service. If you do not bring your payment with you, you may be asked to reschedule your appointment. In the event that you do not have insurance, payment is due on the day of service.
It is the patient’s responsibility to understand their insurance plan and its benefits. We will try to assist you when possible with what is and is not covered. The patient is responsible for services/items that the insurance does not cover.
Once payment is received from your insurance company, you will be billed ONCE for the balance due. Payment is expected within 10 days. SHOULD your account become delinquent, we will be forced to forward it to a Collection Agency. This will also result in discharge of you and your immediate family from the practice. At that time, you will become responsible for ALL of your original charges, also legal fees, collection costs (35%) and interest.
Please pay with cash, credit or debit card. We do not accept checks.
A late fee of $50 is assessed on ALL unpaid balances after 60 days.
