We are given a Substitute folder each year at school. They are purchases and has requested information already printed on the folder to fill in for your classes. We have to fill them out in the first three weeks of the school year or before you have a sub for your room.

Sections on the folder include:

Daily Schedule: Period, Time of class, Monday-Friday (name of class taught)

School Day : When school open time, lunch time and dismissal time for the day.

Extra duties: This is where I jot down Tornado drill information and /Fire drill information.

Lesson plans

Paper/pencilsa

Student Passes/ Referrals

Attendance Book

Handouts

Seating Chart

Other: I usually leave note on loaning out my pens or pencils but not without collateral from student to get it back. Otherwise I have come back cleaned out of pens.

Get Assistance from: I leave one or two teachers names who are nearby and their room number.

Personnel information: I write in name of Principal, Assist. Principal, Dean of Students, Main Secretary, Attendance Clerk and my custodians- their name and their phone extension number

Location of section: Staff Restrooms, staff lounge. Lunchroom/cafeteria and main office

Special instructions: and Attendance Procedure sections

BACKSIDE OF FOLDER:

Procedures: Emergency numbers I list names and extension umbers of Principal, Assist, Principal, Dean of students, nurse, secretary, Attendance clerk and Guidance office secretary.

Severe Weather: I tell where fire and tornado maps are located inclassroom.

Class Discipline: I leave three rules. 1.In seat by bell

2. No profanity

3. Students must bring book, paper and writing utensils daily

Lunch : what time and where to go.

Attendance: I tell them to write on paper and turn in at end of day or to call with those that are absent.

Hall pass rules: I have 3 rules 1.No passes out unless an emergency

2. No student leaves classroom unless they have a signed agenda from sub or the teacher requesting them or guidance.

3. No library passes unless they are finishing a project for me that I have left notes for that student.

I would be very careful about putting the medical info in there without talking to your principal. Yes the sub should know, but I would cover my butt before releasing that to a sub. Here at Ritter that information isn’t even released to the teachers unless there has been a specific issue with a specific student. Just be careful!

The last two items really do have to be included, especially if for the student's safety. Issues such as seizures, allergies etc. must be included regardless of privacy issues. A sub has to know what to do in case of an emergency! the guidelines must be specific. (Been there!!!!)

Students who are good for helping
Classroom procedures
Rules to enforce
Discipline Referrals
Good behavior awards
Disclosure documents
Lesson plans for the day
time-out/behavior intervention sheet
names and extensions or room numbers of other teachers who can help
report forms (how did the day go?)
Procedures: handing in assignments, end of the day tasks, fire alarm, assembly, etc
School map: lunchroom, office, bathroom, teachers lounge
generic daily plan for all periods
extensions for secretaries, school nurse, principal, asst principal
blank class roster
best class of the day contest
numbers for custodian, computer tech specialist
directions for using projectors, dvd players, doc camera, etc

(I know on the last two ideas there are concerns because those are private information. Use your head)
