NYU Steinhardt Business Kit Order Form

Please see your Department Administrator to review the paper choices; or contact Mary Beth Fenlaw in the NYU Steinhardt Dean’s Office. Save this order form to your desktop. You may print it, complete your order information, and fax to Bird at 212 982 6329. Include a separate sheet with the personalized contact information for each item, and any special instructions, printed clearly or typed. You may also email your order by typing in your information to this document. Click in the boxes to indicate your choices. Save all changes and attach this document along with a separate Word document that includes the personalized contact information to appear on each item ordered along with any other special instructions, and email to . You may email or call Rita at 212 982 6329 with any questions about your order. Note: there is a $25 per item set up charge, and there is a $12 shipping charge for each order. You will receive a pdf proof for review via email before your items are printed. Your order will be processed upon receipt of your fax or email proof approval. Your items will be shipped to you via UPS, and you will receive an invoice for your items from Bird, per their price list published on the Business Kit Information page. Bird Advertising, Design & Fine Printers is an NYU approved vendor and has been providing print services to the University for over 25 years.
