FMPPCP Strategic Plan Review
- The revised strategic plan has been forwarded to the Dept of Health.
Position Descriptions for PCP Secretariat staff have been reviewed, performance appraisal to being conducted.
IHP Coordinatior locum position:
- advertising period extended to 27th June
- 4 applications received to date.
New Member Agency
- An orientation visit was conducted with The Village Baxter
Family Violence Help Card for Children & Young People Consultation – 9th June
- consulting the sectors on design and content of the cards – extremely successful
- recommendation to proceed with a card for young people and to explore the ramifications of developing something for children further.

Integrated Health Promotion / Stronger Communities (Respectful Relationships) Project:
- Project Coordinator (Michelle Wright) has hit the ground running and is currently compiling a project plan for the next 12 months.
- A review conducted over the period where there was a vacancy in the position is informing a change to the governance structure which will be discussed at our next meeting on the 27th June.
- We are currently negotiating some changes to the original funding agreement to better reflect how the project is being implemented and extending funding so that the time lost in having a position vacant is regained at the tail end of the project.
Social Inclusion:
- Act Belong Commit forum was held on Tuesday 24th of May, we had over 60 people attend on the day. Findings are currently being compiled however initial feedback has been very positive.
- Needs Assessment emerging themes were presented at the Mornington Peninsula Service Providers Network on Thursday 19th May
- Support is still being provided to put on the ‘Connecting People to Places: Community Transport Solutions’ forum which has been postponed to the 12th of July, it is still planned that this work will be led by the Transport Connections Project post this forum
CapacityBuilding:
- Evaluation of IHP plan, results are still on track to be compiled for the July HPC meeting
- The Preventing Violence Against Women short course was held on Tuesday 31st May and Wednesday 1st June at Mornington Golf Club, Vic Health facilitating this and we had 26 people participate from a variety of PCP member agencies (the course was at capacity)
Service System Redevelopment / SSR Revised Plan Implementation
- Progressing work on Assessment Priority– Better Needs Assessment Project – with LGAs agreeing to run consecutive pilots of their Living at Home Tools which are being benchmarked against the BNA Tool developed and used in PHCH.
- Process/methodology and tools for audits/benchmarking associated with each priority area (Ax, Care Planning, E Health- Referral) are currently being discussed and developed. The audit processes will be undertaken by Juliet Frizzell (JF Consulting) during July – Sept.
PH – CiYC -Integrated Diabetes Service System
- Julie White is coordinating this inititiative. The first meeting of the catchment Working Group is being organised.
- As previously indicated much of this work will lead the PCP towards partial fulfilment of the original SSR plan (2009-2012) to develop a service access model for the catchment. There should be some transferable learning and system improvements from the Integrated Diabetes Service System project
Mental Health
- We continue to work very closely with the SMR Community Mental Health Planning & Service Coordination InitiativeProject Manager and the Mental Health Alliance to strengthen the partnership between the MHA and the PCP.
- Have worked with the MHA Chair on a discussion document which will be used to inform a facilitated MHA workshop to determine future direction & work of the MHA.
- PCP & MHA are hoping to continue to forge close links to capitilise on synergies between the work of each entity.
E Referrals
- This PCP is still needing to substantively increase its referrals. Seeing very little in the way of referrals coming through to/from PH Access.
- We have been steadily increasing our e referrals over the last year and have seen a substantive increase this year to date.

- Due to changes in the security for the use of the PH IT room we can no longer train external agency staff on Connecting care. This is necessitating us to move more quickly to a user pays system and other training facility option.
DH - 2010 Survey Results
- We have been informed by Cheryl Wilson at our last SSRC meeting that the SMR and also this catchment is still falling well below the ‘state average’ in several service coordination areas/ICDM areas – particularly referral practices and care planning. These areas are continuing to be addressed through our SSR Plan priorities.
- They are keen to see a much greater increase in survey participation by all SMR PCP members in the 2011 survey process.
- We have also been encouraged that it is considered we are doing some excellent and innovative initiatives.
