OVERVIEW

Hot work cannot be started until a properly prepared permit is issued to the people doing the work.

Hot work covers the use of any device that produces flame, heat, or sparks. Included but not limited to gas cutting and welding torches, arc welders, blow torches, space heaters, open fires, soldering irons, sandblaster, grinders, electric drills, metal saws, non-explosion proof motor, electrical contacts, internal combustion engines, cameras, vehicles, lighters, cell phones or any device that produces an ignition source.

A hot work permit serves as written permission to do the work, identifies safety checkpoints necessary to make the areas safe for hot work, and alerts operations to the work in progress,

The BP Hot Work Permit will consist of a (5) page form.

1.0 Permit Provisions / __1.1 Verbal permission cannot be given to do hot work. A written permit must be issued, except in shops where there is routine hot work or for vehicles on plant main roadways.
__1.2 The operator performs all tests, fills in all areas on the hot work permit, and signs proper area (Operator). The operating supervisor of the work zone authorizes a permit be issued and signs the proper area (Permit Issued By). The relief operating supervisor and operator assume responsibility for the safety of hot work that extends through a shift change. In so doing, the operator will recheck hot work conditions.
__1.3 A hot work permit can be valid up to 24 hours.
__1.4 The operating supervisor may assign a qualified person to prepare the permit. The operating supervisor and operator are responsible for safe hot work conditions in their zone (area).
__1.5 Both BP’s “contact” (Maintenance Supervisor or Project Engineer, etc.)and the contractor’s agent must sign the form to make it valid for HOT WORK.
__1.6 The hot work permit will also be used when a vehicle entry permit is needed. In the “Nature of Work” section of the hot work permit, write vehicle entry.
NOTE: Only the Operator signature is required for Vehicle Entry Permits.
__1.7 The person performing the hot work must have a proper permit copy before they can start hot work. They must keep this copy until work is completed or the time limit expires.
__1.8 Mechanics are responsible for doing their work safely. They will comply with any restrictions or precautions noted on a permit.
__1.9 The foreman of mechanics will check to see that the work is being
done safety.
__1.10 Operating personnel will check the progress and conduct of the work and take steps if necessary to insure continuing safety.
__1.11 When conditions may make it unsafe to proceed, stop hot work immediately. Resume work only after a recheck and retest shows it is safe to do so.
__1.12 When mechanics complete hot work or must continue past the time limit issued on the permit, they will inform Operations.
__1.13 Before leaving the job site, mechanics will see that hot slag and embers are quenched.
2.0 Preparing Permits / __2.1 The permit book contains one set of forms consisting of five pages. The pages are white, pink, and yellow. “Safety checkpoints” will appear on all copies.
__2.2 Press hard with a ball pen or blunt pencil to make clear copies.
__2.3 Make a check mark for each “safety checkpoint” in either the “O.K.” or “does not apply” space. A space left blank will be taken to mean that an item has not been checked and is unsafe.
__2.4 Record test result, time, date, and type of instrument used for test in test required area. When hot work permit copies have been distributed and a retest must be completed, write test result on operator’s yellow copy and the copy of the person performing the hot work. Record retest result, time and operator signature.
__2.5 The safety measures required for safe hot work are defined by Procedure SAF 044.
__2.6 Hot Work Permits may be completed on the shift, prior to the assigned work day. Testing could be completed and recorded on the Hot Work Permit on the prior shift. Retests may be needed before Hot Work commences.
3.0 Issuing permits / __3.1 Issue the white form to the BP person doing the hot work.
__3.2 Post the pink copy in the area or unit control house as a “work in progress” notice.
__3.3 Keep the yellow copy in the permit book. Label cover of book with unit name and date of first and last permit.
