6- BRIFE RESUME OF THE STUDY:

6.1 NEED FOR THE STUDY

Obesity is one of the burning problems globally as it will hamper the different system of the body. An obese person is always prone to land up in complication like dyslipedemia, hypertension, DM, OA, Infertility, impotency as well as psychological disorders like stress, anxiety, depression etc1.

Obesity has reached epidemic proportions globally, with more than 1 billion adults overweight - at least 300 million of them clinically obese - and is a major contributor to the global burden of chronic disease and disability. Often coexisting in developing countries with under-nutrition, obesity is a complex condition, with serious social and psychological dimensions, affecting virtually all ages and socioeconomic groups.

The obesity epidemic is not restricted to industrialized societies; this increase is often faster in developing countries than in the developed world.

Obesity and overweight pose a major risk for serious diet-related chronic diseases, including type2 diabetes, cardiovascular disease, hypertension and stroke, and certain forms of cancer. The health consequences range from increased risk of premature death, to serious chronic conditions that reduce the overall quality of life. Of special concern is the increasing incidence of child obesity. Approximately 85% of people with diabetes are type 2, and of these, 90% are obese or overweight2.

Global incidence of mortality and morbidity is also highly alarming due to the secondary complication of the Obesity that, controlling the obesity has been considered as the need of the hour. Ayurveda with its holistic approach can help in this condition with its unique way of lifestyle management and the drug therapy. Swasthavritta has a role to play in the management of the Sthaulya.

Many studies have been conducted in the management of Sthaulya by using oral medicines like YavamalakiChurna, TriphalaChurna and Medohara-Vidangadi-Louha, etc; with beneficiary effects... So there is a need to find a suitable drug for the management of Sthaulya effectively.

It is not only a condition of deposition of the fat in the Areolar tissue but it is a condition in which the utilization of the collected fat is hampered. This is the reason why the treatment is highly difficult.

Sthaulya is caused by over intake of Guru, Madhura, and Singdha Ahara, lack of physical exercise, day sleep and Kapha- Meda-Vridhikara Ahara-Vihara3. Sthaulya is explained as ati-Brumhana-JanyaVvyadhi4. A person having extensieve growth in Udaradi region is termed as Sthula and state of Sthula is called sthoulya5

Again Sthaulya is difficult to treat, due to Vata-Kapha involvement which is contradictory to each other6.

Deepana, Pachana,Vatahara, Kapha-Medohara,Teekshana, Ushna, Ruksha Dravyas are advised in management of Sthaulya. As it is one among Shleshmaja-Nanatmaja Vyadhis7, practice of Karshana has to be done in Sthaulya.

Thus Varaasanadi kashaya comprises of Vara (Triphala), Chitraka, Asana, Loha and Haridra, which is mentioned as Sthaulya-hara Kashaya in Sahasra-Yogam8. It is claimed that its use for one month with honey9 as Anupana cures Sthaulya. It is having Teekshana, Ushna, Ruksha, Deepana, Pachana, Kapha-Vatahara properties.

Therefore this Kashaya is selected for this study to evaluate its role in the management of Sthaulya.

6.2 REVIEW OF LITERATURE:

Sthaulyais explained as Santarpajanya vyadhi10. Sthaulya is included in Asshta-nindita Purusha11 which caused by over intake of Guru, Madhura, Shitha, Snigdha Ahara, Avyayama,Divasvapana etc.Chala of Sphik–Stana-Udara, are the important Sthaulya Lakshanas12.

Sthaulya is caused by over intake of Guru, Madhura, Singdha Ahara, lack of physical exercise, day sleep and kapha -Meda-Vridhikara Ahara Vihara. Sthaulya is explained as Athi-Brumhanajanya Vyadhi.

In Sthaulya, persons Ashtadoshas are mentioned whichare Ayushahrasa,Javaporodha, Krichravyvaya, Dourbalya, Dourgandhya, Atisweda, Atikshuda, Atitrishna13.

As Vata, Kapha And Medas are the important vitiated factors in Sthaulya,Vata-Kapha and Medohara Ahara and Aushada as mentioned, is advised14.

Here in Varaasanadi Kashaya, Vara Is Triphala which is mentioned as an important drug in Sthoulya ,Santarpanajanyavyadi and is Tridosha samaka15. Asana(Terminalia tomentosa) with kashaya Tikta Rasa, Tridosha Samaka16 ,Chitraka(Plumbago Zeylenica) and Haridra(Curcuma longa)17 is one among Lekhaniya-Dravya18 and is having Laghu,Ruksha Guna ,Katu Rasa and Vata-Kaphagna property. Amalaki(Emblica officinalis) is Tridoshasamaka19, Haritaki (Teminalia chebula) by Laghu ,Ruksha Guna acts as Sroto-Bandahara and is Medo-Deha-Dourgandyahara20

Lohabhasma is also mentioned in Sthoulya and Vata-Kapha Vyadi with maximum daily dose of 24 Ratti (3 gm) 21.

Most of the drugs are Deepan and Pachan. By virtue of its Deepan and Pachan Karma, the Vara-Asanadi Kashaya works at the level of Agni.By Deepan properties; it mainly corrects the Medo-Agnimandaya and checks the further progression of Medo Sanchaya by preventing the formation of Meda.

All the above said properties acts on Vata-Kapha and Medodhatu and alleviates the same. . Prag-Bhakthakala helps in Karshana .Lekhana drugs are indicated in early morning and before meal22.

Here Madhuis used as an Anupana, Madhu is heavy and non- nourishing (Guru Cha Apatarpanam) and by its Lekhana Karma & Yogavahi nature it helps in yielding better result23.

So here Sthoulyahara Kashaya (varaasanadi) which claims improvement in one month is taken for research.

6.3 OBJECTIVE

To evaluate the effect of Varaasanadi Kashaya in Sthaulya

7 MATERIAL AND METHODS

7.1 SOURCE OF DATA

Patients will be selected from the OPD & IPD of the SDM Ayurveda College Hospital, Hassan.

7.2 METHOD OF COLLECTION OF DATA

Persons fulfilling the diagnostic and inclusive criteria will be selected for the study.

  1. DIAGNOSTIC CRITERIA:

Lakshanas of Sthaulya, as per classics25.

BMI= 30-35 kg/m2

  1. INCLUSION CRITERIA:
  1. Patients between the ages 16 and 60
  2. Patients who are obese with BMI >30 kg/m2
  1. EXCLUSION CRITERIA
  1. BMI <30 kg/m2
  2. BMI >35 kg/m2
  3. Over weight due to secondary causes and with other systemic diseases like DM,hypothyroidism, PCOD etc.

PLAN OF STUDY:

SAMPLING METHOD

Random sampling method

  1. RESEARCH DESIGN

GROUPING:

A total number of 30 samples will be selected randomly and divided in to two groups each comprising of 15 patients each.

Group - A:

In the clinical study 15 patients fulfilling the criteria will be given Varadi Kashaya 60 ml with Madhu, before breakfast for one month. The patients will be assessed weekly.

Group - B:

15 patients fulfilling the inclusion criteria will be given Vrikshamla Kashaya in the dosage of 60 ml in two divided dosage, before food, for a period of 1 month.

SOURCE OF DRUG

The kashaya will be collected genuinely from Shanti Ayurvedics, kannur.

E.ASSESSMENT CRITERIA

1) SUBJECTIVE CRITERIA

  • Decrease in signs & symptoms of Sthaulya.
  • Various features of obesity will be considered and grading will be given to analyze the results statistically.

2) OBJECTIVECRITERIA:

  • BMI
  • Circumference of Chest, Abdomen, Hip, Mid-arm, Mid thigh.
  1. DATA ANALYSIS: The findings of before and after treatment will be recorded and evaluated statistically by using paired and unpaired-test”.
