Goals Activity Sheet 1302.31(b)(1)(iii)

Policy

The Goals Activity Sheet will assist staff to integrate child assessment data in individual and group planning that will include developmentally appropriate learning experiences focused toward achieving progress outlined in the Head Start Early Learning Outcomes Framework.

Procedures

1.  The Goals Activity Sheet KHS 600 will be completed along with weekly lesson plan. This form is to be utilized to provide individualized learning activities. The activities provided on the Goal Activity Sheet can be useful in gathering information for Teaching Strategies GOLD documentation.

2.  You may use an objective level from TSG as a goal or concept to be addressed.

3.  Each day an activity will be presented to meet a specific goal or concept.

4.  Write in the name of the activity, the area it addresses, and the goal or concept of the activity. Write in the initials of the children you are planning to focus on for this activity.

5.  After completing the activity, document what you observed the child doing in the progress notes (example: RS was able to count to 10, CC was able to count to 5).

6.  If activity meets an objective, document in TSG.

7.  Document in progress notes and comments as soon as possible after completing activity.

8.  Every child is to be listed on Goals Activity Sheet at least once per week. A child may be listed multiple times during the week according to the activities provided and his/her need.

9.  A whole class activity may be presented only once per week.

10. Activity may be repeated as needed to meet the needs of the children but may not be repeated in the same week. Activity may be repeated as often as needed but cannot be counted as Goals Activity Sheet requirement.

11. Goals Activity Sheet KHS 600 to be utilized along with Weekly Lesson Plan KHS 602.
