The Department / Located on the ground floor of the hospital, the Department of Radiology
provides leading edge DGH radiology facilities. Although small, its size has
allowed it to develop advanced and innovative solutions to meet ever
increasing demand. Currently the Radiology Department comprises:-
·  2 general rooms including OPG
·  1 digital screening room
·  a self contained Nuclear Medicine Department (Gamma Camera with integrated CT),
·  a purpose built CT/MRI scanning suite
·  3 ultrasound rooms
·  DEXA scanner
A separate digital mammography room is located within the outpatient
department. Most plain films are acquired using a CR system. Reporting is
performed using DICOM workstations for image viewing. The department has
a Carestream Radiology information system and Carestream PACS system.
It is part of the Scottish National PACS . From November 08 all images have
been stored on the National PACS Archive. There are tele-radiology links to
the Consultant Radiologists’ homes allowing remote access to Radiology
images and the hospital network.
In addition, other external drivers are moving the Department forward:-
·  Diagnostic Waiting Times targets
·  the changing pattern of healthcare, including the management of unscheduled care and the move towards 7-day and extended hours working
·  further development of skill mix; radiographers already perform Barium enemas, Barium meals, routine ultrasounds and IV injections
Current activity is in excess of 75,000 investigations a year, including 10,000
CT scans, 4,000 MRI scans and 9,200 ultrasound scans.
There are currently six full time Consultant Radiologists in post. They are Dr John Reid, Dr Hamish McRitchie, Dr. Luis Ferrando, Dr Rachel Thomson, Dr Simon McGurk and Dr Lauren Harris. This post has arisen because of increasing demand and activity in across all imaging but in particular CT and MRI. The Department of Radiology takes part in the South East of Scotland Radiology Training Scheme. There is a Specialist Registrar allocated to the Borders General Hospital on a regular basis.
The Department has close links with primary care and the GP community,
with open access referrals and development of services such as the Borders
Colon Service, an integrated referral system for suspected cases of colon
cancer.
There is a very limited amount of private work, the proceeds from which are
shared equally between the Radiologists.
2.1 Workload
The department performs in excess of 75,000 examinations annually
covering routine Radiological procedures including plain film reporting,
barium Radiology, ultrasound, mammography (non screening), general
contrast procedures Nuclear Medicine, CT and MRI. Cross-sectional imaging
is an increasing proportion of the workload.
2.2 Radiographic and Nursing Staffing
The radiographic and nursing staffing of the main department is headed by
the Superintendent Radiographer, Mrs Lesley Wilson, who is supported by 28
radiographers (24 whole time equivalents), 2 nurses and 6 ATOs. The
Nuclear Medicine Department is staffed by 2 full time radiographers who
perform in-house dispensing duties and are supported by physicist input from
Glasgow.
2.3 Department Office Facilities
A team of 9 clerical staff is headed by our office supervisor, Mrs Anne Bell. In
addition to general office and typing duties, which are provided on a pool
basis, a personal secretarial service is provided for the Consultants by Ms.
Elaine Taylor.
2.4 Office Accommodation
Each Radiologist is provided with a personal office with relevant facilities
including a PACS workstation and access to the Radiology Information
System, the general hospital IT network and the Internet.
2.5 Organisation of Medical Administration
Radiology is currently part of the Diagnostics Division of the Borders General
Hospital together with Laboratory, Endoscopy and Physiological
Measurement. This division is currently led by Dr John O’Donnell
(Consultant Biochemist) with Dr Luis Ferrando being clinical lead for Radiology.
POST INFORMATION
The Post / A whole-time/part-time post has become available in the Department of
Radiology at the Borders General Hospital in Melrose. The successful
applicant will be expected to participate in the core Radiological Service, as
well as take part in the development of new and existing Services as
outlined below, reflecting the Borders Health Board’s commitment to
modernisation of the NHS and the Borders’ Integrated Health Strategy.
This position provides a unique opportunity to assist the Radiology Department in developing new services and forging closer working links with both community health services and tertiary services.
Reporting Arrangements / The Lead Radiologist will agree your job plan on an annual basis. A Consultant Microbiologist (Dr John O’Donnell) is the Head of Diagnostics for Borders General Hospital. Diagnostics comprises Radiology and Laboratory Specialties.
Health and Safety / You are required to comply with NHS Borders Health and Safety Policies.
Training Grade Medical Staff / You will share responsibility for the training and supervision of the Training Grade Medical staff (Foundation Trainees, GPSTs and Specialty Registrars) from various specialties who work with you, and you will be expected to devote time to this on a regular basis.

WORK PROGRAMME AND OUTLINE JOB PLANS
The department is currently reviewing job plans, with the support of hospital
management, to ensure that expected sessional workload is maintained at an acceptable and appropriate level. Once completed, the job plans should provide a robust system for monitoring activity and planning.
It is anticipated that the work pattern of a full time consultant shall be:-
1.  At least 7.5 Programmed activities of procedural and reporting
Radiology, incorporating emergency (on-call) duties
2.  At least 1 SPA, to include non-clinical administration, audit,
Committee work, teaching, continuing medical education and
in-house training.
Notes about the job plan:
·  Job plan will be modified if successful candidate agrees to work less than full time commitments.
·  In the job plan the balance between Direct Patient Care (DCC) and Supporting Professional Activity (SPA) will be the subject of discussion and agreement between NHS Borders and the successful applicant.
·  One SPA is the default for the job plan of new consultants posts. On appointment, or at any time thereafter SPA time can be negotiated for specific, clearly identified duties. These activities would have to be agreed with the individual and acceptable to NHS Borders.
·  Any activities agreed with new consultants must be capable of being performed within the allocated SPA time. Usually at annual job plan review consultants will demonstrate that they are fulfilling their allocated SPA duties.
The details of the workload shall be arranged within the Department of
Radiology and may be altered to accommodate the changing demands
placed upon the department. The Consultant shall be expected to share in
the general work of the department including reporting, barium and contrast
work, ultrasound, non-specialised Nuclear Medicine, CT and MRI.
Job plans are agreed in association with the Lead Radiologist and Head of Clinical Service and submitted to the Associate Medical Director and General Manager for approval. Changes will be discussed and agreed by these officers and yourself in line with service needs and at an annual review.
5.1 On Call
On call duties shall be on a one in six basis with prospective cover when
colleagues are on annual/study/sick leave. On call duties shall include
general Radiological services, as agreed within departmental operational
policies.
There are no junior Radiology staff on the on-call rota. A 5% availability
supplement, calculated on the basis of full time basic salary, is paid for
participation in the 1 in 6 on-call rota, which will become a 1 in 7 on call-rota
upon filling this post.
5.2 Specialist interests
The department encourages and supports consultant specialist interests.
Cross-sectional imaging is the likely area of potential expansion. However,
other areas of interest would be welcomed and accommodated as far as
possible.
5.4 Academic
The Radiology Department holds regular joint meetings with the Physicians, Surgeons, Paediatricians and Orthopaedic Surgeons. There is a regular commitment to teaching medical students and a hospital education programme taking place on a half day monthly, on a rolling basis. In addition to this, continuing medical education and in-house training are encouraged. Opportunities also exist for audit and research. Good audiovisual facilities are available within the department.

This is an outline job plan and the job plan will be agreed and revised if necessary with the successful candidate. Final job plans are agreed in association with the Head of Clinical Service on at least an annual basis. Developments and changes will be discussed and agreed in line with service needs. Borders General Hospital
The Borders General Hospital is the district general hospital serving the Scottish Borders Region. It has 320 beds with 87 acute medical beds of which 12 are in a higher dependency area with monitoring and telemetry facilities. The hospital offers services in General Medicine, Haematology, Palliative Care, Medicine for the Elderly, Stroke Medicine, Paediatrics, General Surgery, Orthopaedic Surgery, Ophthalmology, ENT, Obstetrics and Gynaecology. There is a 6 bedded ITU and an active outreach programme for critically patients. A very recent development is the opening of the Margaret Kerr Unit in January 2013 has provided palliative care services in the Scottish Borders with a purpose-built environment from which to deliver specialist palliative care.
Within the Department of General Medicine there are specialists in Cardiology, Respiratory Medicine, Diabetes and Endocrinology, Gastroenterology, Neurology and Rheumatology. Visiting consultants provide clinics for Oncology, Dermatology, Renal Medicine and Clinical Genetics.
The Emergency Department provides facilities for the reception, resuscitation, examination and treatment of patients in the Scottish Borders who require emergency admission, or immediate care and discharge. The ED service is well integrated with the Borders Emergency Care Service (BECS), which is the Primary Care out-of-hours service for Borders. The department is managed by a consultant emergency physician, Mr J Kerr, who was appointed in November 2008. Specialty Doctors, Salaried GPs, training grade doctors (FY2s, GP, orthopaedic and surgical trainees) manage ED patients supported by speciality medical staff from within the hospital. Nurse staffing within the ED is comprised of 23 registered nurses and 3 nursing assistants led by a senior charge nurse.
There is a 6 bedded ITU run by the Anaesthetic Department who also run an acute haemodynamic support service with outreach staff from ITU liasing with ward staff.
There is an up to date laboratory service covering Haematology, Microbiology and Clinical Chemistry with ready access to more specialised investigations in Lothian laboratories and other laboratories in Scotland. The Radiology Department is well equipped for routine radiology, CT scanning, ultrasound, Doppler studies, nuclear medicine and MRI. CT coronary angiography and cardiac MRI are well established in the department.
