Students with a Diagnosis with an Autism Spectrum Disorder
· Refer to the Health and Human Resources Commission for the Medicaid Waiver Program, Community Living Assistance and Support Services (CLASS). PH: 512- 438-5658
· Refer to Star KIDS for the Medicaid Waiver program, Medically Dependent Children’s Program (MDCP). PH: 512-438-5658
· Refer to the Harris Center for Mental Health and IDD to complete the (1) intake interview (2) complete the “Determination of Intellectual Disability” (DID) eligibility evaluation and (3) place child’s name on the Medicaid Waver Wait List for Home and Community Based Services (HCS).

Refer to the Harris Center for Mental Health and IDD to complete the (1) place child’s name on the Medicaid Waver Wait List for Home and Community Based Services (HCS). Students with a Diagnosis of Significant Medical Condition, Genetic Disorder, Medically Fragile, Other Health Impairment, or Orthopedic Impairment

For Students with a Diagnosis of Emotional Disturbance
· Refer to Harris Center for Mental Health and IDD / Mental Health Division for psychiatric/ medical, case management, and transition services. PH: 713-970-7000.
· Refer to YES (Youth Empowerment Services) Waiver a Medicaid Waiver that allows for more flexibility in the funding of intensive community based services for children/adolescents ages 3-18 with severe emotional disturbances and their families. PH: 713-970-7212.

Social Security Supplemental Income/SSI
· Refer to Social Security Administration to schedule a phone interview to initiate an application for Social Security Supplemental Income (SSI) benefit. PH: 1-800-772-1213
o Appropriate referral for families with children currently receiving Medicaid or CHIPS
o Appropriate referral for families on free or reduced lunches.
