Introduction

This implementation strategy describes how Kahi Mohala, a Sutter Health affiliate, plans to address significant needs identified in the Community Health Needs Assessment(CHNA) publishedby the hospitalonOctober 25, 2013. The documentdescribeshow the hospital plans to address identified needs in calendar (tax) years 2013 through 2015.

The 2013CHNA and this implementation strategywere undertaken by the hospital to understand and address community health needs, and in accordance with proposed Internal Revenue Service (IRS) regulations pursuant to the Patient Protection and Affordable Care Act of 2010.

This implementation strategy addresses the significant community health needs described in the CHNA that the hospital plans to address in whole or in part. The hospital reserves the right to amend this implementation strategy as circumstances warrant. For example, certain needs may become more pronounced and merit enhancements to the described strategic initiatives. Alternately, other organizations in the community may decide to address certain community health needs, and the hospital may amend its strategies and refocus on other identified significant health needs. Beyond the initiatives and programs described herein, the hospital is addressing some of these needs simply by providing health care to the community, regardless of ability to pay.

This implementation strategy describes how Kahi Mohala plans to address significant health needs identified in its 2013 Community Health Needs Assessment and consistent with its charitable mission. The strategy describes:

  • Actions the hospital intends to take, including programs and resources it plans to commit;
  • Anticipated impacts of these actions and a plan to evaluate impact; and
  • Any planned collaboration between the hospital and other organizations.

Mental Health & Mental Disorders/Substance Abuse & Lifestyle

Name of Program, Initiative or Activity / Crisis Response Pilot Project for Emergency Room Patients
Description
/ The pilot project will provide an experienced behavioral health professional to Leeward and Central Oahu hospitals and emergency rooms, such as Wahiawa General Hospital, Pali Momi Medical Center and others, to assess, evaluate and de-escalate psychiatric patients in crisis. The behavioral health evaluator will be a ‘community connector’ to recommend community resources and placement.
The behavioral health professional will also provide training to emergency room staff in de-escalation techniques and management of behavioral health patients. Training topics may include how to recognize signs, symptoms, and engage in practical techniques for patients exhibiting behavioral health issues (such as substance abuse, suicidal ideation, and trauma). The trainings will be offered at the most convenient location for emergency room staff, either at their hospital or on the KahiMohala campus.
Anticipated Impact and Plan to Evaluate
/ The pilot program is expected to reduce average length of time that a patient who is presenting a primary psychiatric condition is in the emergency room; accelerate the response time for an emergency room patient to be evaluated by a psychiatric professional; deliver 40 hours of capacity building training; produce positive outcomes for 500 behavioral health patients in crisis over the course of a 12 month period.

/ Community Health Center Care Coordination Program
Description
/ Establish a single point of contact between a Kahi Mohala case manager and Waianae Coast Comprehensive Health Center (WCCHC) aftercare coordinator to improve communication with the patient’s primary care physician, to more effectively schedule aftercare appointments, and to ensure a seamless discharge fromKahi Mohala.
Anticipated Impact and Plan to Evaluate
/ We anticipate that the program will result better coordination of care between our hospital and the WCCHC primary care provider. We will monitor the impact by jointly tracking the percentage of patients who receive a medication management consultation within 30 days of discharge from Kahi Mohala and tracking the 30-day readmission rate for Leeward Medicaid patients.

No hospital can address all of the health needs present in its community.
