Field Trip Permission Form

Dear Parents:
The fifth grade students will be attending a performance by the Baltimore Symphony Orchestra at the Meyerhoff Symphony Hall in Baltimore on Wednesday, February 2, 2011 at 10:00a.m. This year we will be enjoying a performance of Beethoven: A Musical Hero. Immediately following the show the students will visit the National Aquarium in Baltimore which includes the Dolphin Show. This trip will be taken on a properly insured carrier. Transportation for this trip will be provided by Brook’s Transportation Services.
We will be departing from Sunset Elementary at 8:45a.m. promptly and returning at approximately 2:00 p.m.
The total cost of the trip is $11.00 per student. This amount includes admission fees and transportation. Students are encouraged to wear dress clothes and comfortable shoes to the concert. Students must bring a bag lunch (no glass containers).
We will need parent chaperones. Please indicate below if interestedin chaperoning. If selected to chaperone, you will be contacted prior to the trip. Interested chaperones do not remit payment unless contacted.
Money and permission slips for students are due no later than Monday, January 24, 2011. Please make checks payable to Sunset Elementary School. This is a wonderful opportunity for our students to experience the arts at a reasonable cost. Thank you for your prompt attention to this matter. Please contact me if you have any questions.

Please be advised that ALL field trips are subject to cancellation AT ANY TIME by the Board of Education, the Superintendent of Schools or the Superintendent’s designee when, in their sole discretion, cancellation is in the best interests of students and staff. In such cases, parents and students bear the risk of loss for financial or other commitments they have made. The Board of Education, its employees and agents will not be responsible for any losses arising out of cancellations.
