POSITION TITLE:Care Advisor

RESPONSIBLE TO: Home Care Packages Program Team Leader

HOURS: Full-Time, 7.6 hours per week, 5days per week

LOCATION:Australian Multicultural Community Services, Suite 111,

44 - 56 Hampstead Road, Maidstone, Vic 3012

AMCS is an established community based organisation with over thirty years of experience in providing support and welfare services to Victoria’s multicultural communities. AMCS has an active role in the provision of community based aged services, is a state wide organisation and is located in Melbourne’s Western Region.

SUMMARY OF POSITION

This autonomous role wil focus on writing detailed case notes and identifying tailored care solutions to enable our clients to achieve their golas and enhance their quality of life. You will aim to make a positive difference to familiies, achieve clients satisfaction and build professional relationships internally and externally.

You will also oversee clients package budgets as well as connecting professionally with family members andexternal organisations to serve the client’s best interests. You will be regularly travelling to meet clients in the community and laising with the extended internal and external contacts to ensure the smooth delivery of care services.

KEY SELECTION CRITERIA

Essential:

  • Degree in Social Work, Nursing, Diploma in Community Services, Case Management or equivalent is essential;
  • Relevant experience in the planning, co-ordination and delivery of services to the frail aged and persons with disability in their own homes incl. care management skills, assessment, care planning and coordination and conflict management;
  • Knowledge of an additional language and an understanding of the cultural barriers faced by the elderly preferred but not necessary;
  • Demonstrated ability to prioritise, plan and organise a busy workload;
  • Demonstrated experience in budget monitoring;
  • Experience in supportingclients with dementia;
  • High level of motivation and a team orientated approach;
  • A high level of professionalism and interpersonal skills;
  • Ability to show leadership and initiative;
  • Ability to appreciate that all Clients have the right to respect and confidentiality;
  • Ability to work in a team environment.
  • Advanced communication and negotiation skills;
  • Demonstrated decision making and analytical skills;
  • A current Victorian driver’s license.

KEY RESPONSIBILITIES

The Care Advisor will:

•Act as the key contact person and advocate for clients in facilitating the identification of goals and development of Care Plan

•In partnership with the client,develop a goal directed care plan, organise services, provide support and monitor services to ensure quality

•Respond to client’s assessed individual needs and preferences within budget

•Consider creative options and solutions that deliver exceptional client outcomes.

•Ensure that the delivery of services meet all associated funding agreement, legal standards and requirements.

•Maintain computer based and paper file documentation of assessments and care plans

•Monitor and maintain records of expenditure and ensure clients plans do not exceed budget allocations

•Assist in the recruitment and orientation of Support Workers as required by Team Leader

•Assist in conducting annual performance appraisals

•Maintain currency of skills and knowledge relevant to this position, including managing clients with complex needs and clients with special needs as identified in the Aged Care Act.

•Liaise with referring agencies, ACAS, local health provider and community services

•Represent AMCS at conferences, community forums and network meeting where appropriate

•After hours On Call service on a rotating basis within the HCPP team

CONDITIONS OF EMPLOYMENT

Continuous employment with the agency is conditional to the receipt of funding for this position.

All employees are subject to satisfactory completion of a six (6) month qualifying period and acceptance by the employer of a current police check. The employer withhold the right to extend the probation period if required.
