SAFETY GLASS POLICY
RESOLUTION NO: 05-145
BE IT RESOLVED, that the Housing Authority of the City of Anytown adopts the following safety glass policy. Any employee working under conditions that constitute a risk to the eyes are encouraged to obtain safety glasses. Eligibility for participate in the safety glasses program will be determined by the Housing Authority Executive Director.
1. The employee will contact the City’s Human Resource department and state that they are a Housing employee. The employee will receive an industrial form and cover letter, explaining the policy to present to the doctor of his choice. The Housing Division director must approve this form. At the same time, a “Verification of the Receipt of Safety Glasses” form will be given to the employee to be signed and returned to Human Resources when the employee has actually received his safety glasses.
2. The employee will be responsible for their own eye examination and subsequent service (including complete frame measurements, verification of prescription, dispensing, etc.).
3. The Authority will pay ShopKo directly for the cost of safety glasses (glass) and specified frames, which must meet all specifications of ANSI Z-87, 1-1978 and OSHA standards.
4. The Authority will provide plain glass safety glasses with any fitting costs to be paid by the employee. Authorization for glasses will be obtained from the Authority.
5. The Authority’s share of expenses will include payment for glass lenses for single vision, bifocals or trifocals and for the following standard frames: Bausch and Lamb, Style X30, Y80, or T30; American Optical, Style P9800; or Bausch and Lamb, Style 7, 307 with permanent side shields. Any expenses beyond the lenses and standard frames listed will be the responsibility of the employee.
6. Nor more than one pair of glasses will be provided every two years, unless glasses are broken due to working conditions during the two-year period.
7. Those employees participating in the safety glasses program will be expected to wear safety glasses at all times while on the job.
