4: / CDM/SC BUSINESS ARISING
4.1 WPCP would like to conduct Service Coordination Professional Development sessions and hope to have a presentation from LCHS (and others) on what they’re doing in this area.
Action: A presenter to be organised for next meeting. It was suggested that this might be someone from LCHS re Stay Healthy Latrobe
4.2 Report against deliverables – this will be now maintained and presented via the Operational Plans. A report card set up will be devised and implemented for next year. / Jo

4.3 S2S meeting – a kit for agency orientation has been created and will be finalised in January.

SESSION 3 – INTEGRATED HEALTH PROMOTION

Network has been formed and to meet on a bi monthly basis. Meetings to be action-orientated and an opportunity to share resources. Terms of Reference have been drafted and to be discussed at tomorrow’s meeting. DoH evaluation portfolios also to be discussed. Approx 20 attended first meeting. Rachael & Jo have attended Introduction to Health Promotion Facilitator Training. Health Promotion Training is a four hour session and gives an overview on the core elements of Health Promotion. The program is adaptable for staff orientation purposes and other contexts /
