Dear Parents/Guardians,

The Child Health Examination Code of Illinois, states that a new physical examination and any needed immunizations shall be required of all pupils who will enter 6th grade, pre-kindergarten or kindergarten during the next school year 2015-2016.

To comply with this law, a health examination is required for your child. This health examination includes both the physical report, the completion of any needed immunizations and the parent’s report of the students’ health history. All students in grades K-12 must show proof of having had two doses of MMR with the first doses being on or after their first birthday, and the second dose no less than 4 weeks later. A COMPLETED series of 3 Hepatitis B immunizations are required for students entering pre-kindergarten and 6th grade- 12th grade. The Pneumococcal vaccine (four doses) is required for students in pre-kindergarten. In addition, students entering kindergarten and 6th grade must show proof of having received two doses of Varicella (chicken pox) vaccine, or having had the disease. All 6th grade students must show proof of having 1 dose of Tdap vaccine and 1 dose of Meningococcal conjugate vaccine (on or after the age of 11 years) and prior to registration. A screening for Diabetes is also required for all students.

Your child WILL NOT BE ALLOWED TO REGISTER FOR SCHOOL WITHOUT THE ABOVE REQUIREMENTS.

Please provide proof of all present and past immunizations at registration. If you have changed physicians, your new physician will require proof of past immunizations prior to administering any new immunizations.

The completed forms are to be returned to your child’s school as soon as possible prior to registration. Take any completed forms to the school office, in an envelope marked for the nurse. Please contact your doctor and dentist to arrange for an appointment now, which will make it much easier than later on in the year.
