Cleghorn Program Referral Information (revised January 2013)

/ Early Psychosis Intervention Program (EPI)[1] / Psychotic Disorders Team (PDT) /
Eligibility Criteria
Age / 14 to 35 years of age.
(some flexibility on age on a case-by-case basis.) / 16 to 65 years of age.
Symptoms / Symptoms of first episode psychosis.
(not limited to specific diagnoses.) / Symptoms of psychosis - may have experienced many episodes of psychosis.
Treatment history / Six months or less of treatment for psychosis. / May have had many years of treatment.
Catchment / Individuals living in City of Hamilton.
EPI partner agencies across the LHIN provide EPI services in the following communities:

*At the request of the EPI partner agencies, Cleghorn Hamilton will provide consultations to their clients or residents of the LHIN outside of their catchment areas. / Individuals living in the West end of the City of Hamilton.

Exclusionary Criteria
Developmental Disability / Developmental Disability & IQ above 75:
·  Will accept with DS sector partners through CONTACT/DSO.
Developmental Disability & IQ 75 and below:
·  Redirect to CONTACT/DSO/Dual Diagnosis Program. / Developmental Disability & IQ above 75:
·  Will accept with DS sector partners through CONTACT/DSO.
Developmental Disability & IQ 75 and below:
·  Redirect to CONTACT/DSO/Dual Diagnosis Program.
Prioritization
Priority Level *All EPI referrals take priority over PDT referrals / 1.  Referrals from PES that can divert an inpatient admission are highest priority.
2.  Referrals from GPs/FHTs and PES when the individual has had no prior mental health treatment are given priority. / 1.  Referred individuals who have no other mental health service provider are given priority.
Additional Information
Response Time / 1.  Intake staff respond to referral source and individual referred within 72 hours.

Information Required for Referrals from the Community / 1.  Completed EPI Referral Form.
2.  A consultation/assessment that was completed within 1 month of the referral, describing chief concerns.
3.  Additional relevant mental health history and consultation notes. / 1.  Completed PDT Referral Form.

Information Required for Referrals from Inpatient Units / The referral should be made before discharge.
1.  Completed EPI Referral Form.
2.  PES notes - Part A and B.
3.  Admission Note.
4.  Medication List.
5.  If a long hospitalization, the most recent MSE/Assessment notes.
6.  On discharge, send Discharge Face Sheet and Discharge Summary Note.
If referred after discharge, all of the above must be included with referral. / The referral should be made before discharge.

Engagement Strategies / Staff will come to visit the referred individual while they are still on the inpatient unit to help build engagement and increase the likelihood of follow-up.
