Authority: The authority to request this information is contained in: 5 USC § 301 (Authorizing Forms and Regulations).
Principal Purpose(s): Agreement signed by the Naval Reserve Officers Training Corps (NROTC) Midshipman. This agreement states the required service commitment and the obligations incurred by the Midshipman for continued scholarship benefits.
Routine Use(s): Information you provide in this application is protected by the Privacy Act and will not be released outside the Department of Defense without your permission unless it comes within an exception to the Act or one of the routine uses in 32 CFR § 701.112, http://www.privacy.navv.mil and the routine uses set forth here.
Disclosure: Failure to provide the requested information may result in removal from the NROTC program and/orloss of scholarship benefits.
1. This is to certify that I have reviewed and understand my Naval Reserve Officers Training Corps (NROTC) Scholarship Service Agreement effective as of the date of this document.
2. Upon entering the sophomore year of naval science and accepting benefits, I understand I am obligated to the U.S. Government. If I do not complete the educational requirements identified in paragraphs 1.81-2 of the NROTC Scholarship Service Agreement (NSTC 1533/135), or if I complete the course but decline to accept a commission when offered, I may be ordered to active service by the Secretary of the Navy to
serve in my enlisted grade or rate for four years or for such lesser period as the Secretary may prescribe. Currently, disenrollees are called for two years. I further understand that if the Secretary of the Navy's decision is recoupment, instead of active enlisted service, I will be ordered to pay back all tuition, book stipend and fees beginning with the freshman year and thereafter.
