60.3 – States that Form OF-346 will be the standard form used as an Operator Identification Card.

60.4 – Assigns responsibility to Unit Fleet Managers to identify employees qualified to operate government-owned or leased motorized equipment and to document and maintain this information.

61.2 – Requires employees to have OF-346 in possession to operate motorized and/or specialized equipment.

62.1 – Safety Coordinator/Committee will assist in developing and coordinating training program for specialized equipment.

65 – Employees will not operate Government-owned or leased motorized fleet equipment without the proper Operator ID Card endorsements. Employees should also contact the Unit Fleet Manager to begin process to renew their OF-346 Operator ID Card before their card has expired.

60.3 – Policy

Form OF-346 will be used for Operator Identification Cards and will list all motorized equipment and/or specialized equipment an employee is qualified to operate.

60.4 - Responsibility

Unit Fleet Managers are responsible for:

1. Identifying employees qualified to operate government-owned or leased motorized fleet equipment.

2. Preparing and tracking, Operator Identification Cards issued to Chippewa National Forest (NF) employees.

3. Documenting employee training and equipment qualifications, and the maintenance of these files under file code 7130-6.

61.2 – Identification Card or Document

Employees will have Form OF-346 in their possession while operating Government-owned or leased motor vehicles and special equipment. OF-346s will expire on the same day as the employee’s State Motor Vehicle License, every four years.

62 – Training

62.1 – Training for Qualification

The forest Safety Coordinator and/or the Unit Safety Committee Member will,

1. Locate qualified instructors,

2. Assist in developing operator-training programs (include Job Hazard Analyzes), and

3. Coordinate the scheduling of training programs for specialized equipment.

Qualified Trainers will provide Unit Fleet Managers with documented proof of employee’s attendance and completion of all training requirements.

65 – EMPLOYEE RESPONSIBILITIES

Employees will not operate Government-owned or leased motorized fleet equipment without the proper Operator ID Card endorsements. Employees should contact their Unit Fleet Manager before their State Motor Vehicle License expires to begin the process of renewing their OF-346 Operator Identification Card.
