PROCEDURES FOR RECORDING Fall 2011-12 RELATED SERVICE ATTENDANCE

The card is a legal record and must be an accurate statement of the related services provided to a student. A separate card must be created for each related service, different group size and provider. The service provider is responsible for recording daily attendance on this card. The card must be maintained in a secure location. All entries must be made in blue or black ink. Any corrections must be initialed in red ink. The use of correction fluid products is strictly prohibited. Instructions must be provided to any staff member completing attendance cards. School Information:

a.  School Name – Important Steps, Inc.

b.  School Code # - Enter “C169” assigned to the site by the New York City Board of Education.

c.  Related Service Provider’s Name – Enter the name of the related service provider.

d.  Type of related Service – Enter the type of related service as it appears on the student’s IEP.

e.  Frequency/Duration/Group Size – Enter the frequency, duration and group size for the related service provided as it appears on the IEP and NYCDOE contract.

Enter information as requested.

Daily Attendance: OT / PT/ ST/ Counseling

Fall Related Services sessions are provided between Sept. 6th , 2011 and June 21st , 2012

Place an “X” for each session received/provided.

For missed sessions use the following codes (explain the reason on the back of the card):

·  A- Child was absent

·  P- Provider was absent

·  C- School was closed

Sessions Missed / Sessions Provided
P / S=A+C

Each month the total of sessions missed and provided must be entered.

Sessions Missed = Total number of sessions missed by the provider (P) and the student (S).

Sessions Provided = Total number of sessions provided during the month.

Admission/Discharge

a.  Date Admitted –All Fall 2011-12 cards must indicate the student’s first attending date in Sept. 2011 or the later date eligible to attend but legally absent.

b.  Date Discharged – Enter the date of discharge only if the student is discharged from the program prior to the last day of the program. Do not enter a discharge date for students who remain in attendance at the program through the last day of class.

c.  Reason – Provide an explanation, as appropriate, for the discharge.

Related service cards must be signed and dated at the time service is terminated or when the service period ends. Cards must be retained by the school in accordance with applicable contractual obligations.
