Gather and analyse data from the Transforming MND Care audit for (1) care centres/networks and (2) other services not funded by the Association / Support the Newcastle and Middlesbrough Care Centres to complete the audit as part of their contractual obligations

RDP Outcome 2: Driving service improvement
Create and implement improvement plans following the use of the Transforming MND Care audit / Work with Newcastle and Middlesbrough Care Centres to implement the audit improvement plans as required

Use influencing and negotiation to improve health and social care services not funded by the Association where there are identified shortfalls / Utilise LDT meetings and wider consultation to help identify shortfalls/inform local clinical and social care priorities, and work with regional staff and HSCPs to influence better provision.
Target commissioners where there appear to be fundamental shortfalls in provision.
In particular:
Patient respite and carers support
  • Map the current availability of respite care for pwMND locally
  • Map the current support available for carers locally
Paid Carers
  • Map/log local issues to bring back to LDT, focusing on particularly isolated and rural areas e.g. Cumbria
Psychological & Emotional Support
  • Improve access to psychological support /develop a referral pathway to neuropsychological services in Newcastle (working in collaboration with existing hospice services?)
Respiratory Pathway
  • Improve access to community respiratory services pathways in line with NICE guideline/Transforming MND Care audit tool (timely). Middlesbrough and Newcastle (including Cumbria)

RDP Outcome 3: Protecting and extending care and support
Manage the contract and relationships with care centres and networks / Work collaboratively with Newcastle and Middlesbrough Care Centres to progress developments in clinical care services and pathways
Maintain membership of Care Centre Steering Groups and develop greater strategic focus, including attendance from CCG commissioners and NHS Trust Business Managers as appropriate. Refresh Terms of Reference.

Secure the engagement of neurologists outside of Newcastle and Middlesbrough Care centres to ensure appropriate referrals are made
Monitor existing referral pathways, analyse where the number of pwMND known to the Association does not correlate to expected prevalence and take action where appropriate

Recruitment, retention and succession planning of all regionally based volunteers / Review and identify volunteer role requirements for each local Branch and Group, specific to local need
