Use the following scenario to complete this assignment.

Patient Scenario

You are a nurse working on a telemetry unit. You arrive for your night shift at 1900 and receive report from the day nurse, Jim. He tells you that your patient, Mrs. G., a 32-year-old woman, came to the ER at 0600 today complaining of leg pain. She has been diagnosed with a large DVT in her right leg. She has no health problems, takes no medications, has no allergies, and is not pregnant. She received an IV bolus of heparin of 5,000 units (from the infusion bag) in the ER at 0800 over 30 minutes, and is being maintained on a heparin infusion. The dose of IV heparin is 20 units/KG/hour. The infusion is in the standard concentration of 25,000 units in 500ml of D5W. Mrs. G. weighs 60KG.

As you enter Mrs. G's room you hear the IV pump alarming and you note that the alarm is informing you that only 50ml remain in the bag. You believe there may have been a medication error. You would expect several more hours before the bag needed to be changed. Describe what actions you would take. If an error did occur, there are at least four critical actions you must take, name them. If you believe there was no error, explain.

Write a 300- to 500-word paper describing the following items based on the patient scenario:

·  Identify if a medication error occurred or not, and state why.

·  State how many hours the infusion should have run and show your math.

·  If you believe a medication error did occur, there are at least four critical actions you must take. List them and explain why.

·  Be sure to include references for your rationale and proper dosage.

Use the rubric provided below as you write your paper. When you think your paper is finished, self-grade your paper using the rubric to gauge your progress.

Include a title page and a reference page. This is not a research paper, so an abstract, introduction, and conclusion are not required.
